A Community Health Worker (CHW) is a public health worker who has a heightened understanding of the community served through their specific program. Depending on clinic priorities or seasons, the specific program the CHW works under may vary. This understanding enables the CHW to serve as a liaison, link and educator between services and the community to promote services and ultimately enhance community health. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy. Our successful candidate will engage in patient identification and outreach, program implementation and patient education. This CHW will currently focus on a variety of Preventive Health Topics such as cancer prevention health programs, dental services, reproductive/sexual health programs, and more.

Position Requirements

Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

 

  • Bilingual in English and Spanish required.
  • Proven ability to maintain professional conduct, including appropriate boundaries required.
  • Able to multitask, develop organizational structures and set goals required.
  • Able to work autonomously as well as collaboratively with all team members required.
  • Able to exercise empathy and understanding required.
  • Able to communicate and listen effectively required.
  • Prior experience working with target population required.
  • Completion of the CHW certification program preferred. Must be completed within 90 days of employment.

Our growing list of services includes adult and pediatric primary care, chronic disease management, wellness and nutrition, behavioral health counseling, substance use counseling, case management, a pharmacy, and more. For medically underserved populations, our one-stop-shop healthcare model reduces barriers to care and increases the likelihood of maintaining long-term health. HOPES accepts most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, we offer discounted services based on a sliding fee scale.

Salary Range $20.50/hr - $22.75/hr
