The Chronic Disease Team at Northern Nevada HOPES has an exciting opportunity for Spanish-speaking Medical Assistants who are looking for a new challenge! Our highly-specialized team utilizes their MA skills to independently provide important disease management services and their CHW skills to educate individuals and small groups. We serve patients of all ages who have been diagnosed with serious chronic diseases like diabetes, hypertension, and childhood obesity.  On any given day YOU could be meeting in the morning with a newly diagnosed patient to discuss the services that our team offers, making contact with a patient whose condition is getting worse by lunch, and teaching a child and their parents about healthy lifestyle changes that would greatly improve their wellness and reduce health risks in the future. Our MA-CHWs make a difference in their patients' lives everyday!

A Community Health Worker (CHW) is a public health worker who has a heightened understanding of the community served through their specific program. Depending on clinic priorities or seasons, the specific program the CHW works under may vary. This understanding enables the CHW to serve as a liaison, link and educator between services and the community to promote services and ultimately enhance community health. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy. Our successful candidate will engage in patient identification and outreach, program implementation and patient education. This CHW will currently focus on outreach and patient engagement for diabetes and hypertension.

Position Requirements

Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

 

  • 5x8s schedule
  • Bilingual in Spanish required
  • Completion of Accredited Medical Assistant Program required.
  • Proven ability to maintain professional conduct, including appropriate boundaries required.
  • Able to multitask, develop organizational structures and set goals required.
  • Able to work autonomously as well as collaboratively with all team members required.
  • Able to exercise empathy and understanding required.
  • Able to communicate and listen effectively required.
  • Valid BLS required prior to hire, per HRSA compliance.

  • Prior experience working with target population preferred
  • Completion of HOPES sponsored CHW-I certification program within 90 days of employment.

Salary Range $20.50/hr - $22.75/hr
