Seeking a Patient-focused and Solution-oriented People-leader to support HOPES expansion!

The primary purpose of the Patient Services Coordinator is to ensure that patient experience is optimized.

The Patient Services Coordinator works with patients and their families to address concerns or special needs that may arise.  This Patient Services Coordinator works directly with various members of the healthcare team, organizational departments, and administration to solve problems that patients and families may have with the goal of facilitating a positive patient experience during their time in the organization. The Patient Services Coordinator acts as an intermediary between service centers and the patient and family to bring about the most effective healthcare treatment and positive experience.

  • We are a Federally Qualified Health Center (FQHC). The mission of all FQHCs is to provide excellent health care services to otherwise under-served populations. We accept most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, we offer discounted services based on a sliding fee scale.
  • We are a National Health Service Corp site, and our providers are eligible to apply toward a federal loan forgiveness program

Position Requirements

* Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

  • Two-years’ experience within a medical arena, especially providing customer service, intake and eligibility, or receptionices required.
  • One to two years working with the underserved, those experiencing homelessness, substance abuse problems and/or mental health issues and HIV-infected persons required.
  • Excellent communication skills, both oral and written required.
  • Ability to use Microsoft Office programs, (especially Excel) and electronic medical records required.
  • Bilingual Spanish/English required.
  • Valid BLS required prior to hire, per HRSA compliance.
  • One to two years of supervisory experience preferred.
  • Excellent problem-solving skills
  • Bachelor’s degree in social work or related field or equivalent combination of education and experience preferred.
  • Knowledge of needs assessment and acuity scales. 
  • Knowledge of state, county and local benefit programs and requirements.  
  • Understanding of harm reduction model of service delivery preferred
  • Experience appropriately handling sensitive and confidential information and situations

Salary Range $44,500 - $52,400 per annum
