Women’s Health Connection Navigator

The Women’s Health Connection (WHC) Navigator is the bridge between patients and their healthcare. This Navigator identifies barriers to cervical and breast cancer screening and helps reduce health care disparities. This navigator assists patients, makes appointments, assures their attendance, and connects them to support services.

  • Patient Contact-Identify patients eligible for the WHC program and establish rapport. Offer education and advocacy. Schedule cervical and mammogram screenings. Seek ways to promote and educate women for optimal health.
  • Documentation-Document all pertinent information and interactions in the electronic medical record. Inform providers and communicate within HIPAA standards.
  • Continuous Learning-As part of the MA team and integrated health team, you’ll learn, teach and engage in continuous learning. You will seek opportunities to stay abreast of women’s health and generate ways to increase learning for patients and providers.

  • We are a Federally Qualified Health Center (FQHC). The mission of all FQHCs is to provide excellent health care services to otherwise under-served populations. We accept most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, we offer discounted services based on a sliding fee scale.
  • We are a National Health Service Corp site, and our providers are eligible to apply toward a federal loan forgiveness program

The WHC Navigator ensures optimal opportunities for women’s health by coordinating cervical and mammography screenings, providing educational opportunities and advocating for patients. The WHC Navigator guides patients through the complexities of the healthcare system and provides support. This navigator assists patients, makes appointments, assuring their attendance, and connecting them to support services.

Position Requirements

Use your patient-focus, communication skills, creativity and flexibility to assist patients to reach their optimal health. Your ability to express empathy and take initiative is a must have!

We are looking for:

  • Bilingual Spanish/English required
  • At least 2-years experience working in a healthcare environment in educational programming and patient interaction required.
  • Knowledge of Microsoft Office applications and basic ability to create graphics and visual presentations; ability to enter information into the electronic medical record and search internet required.
  • Excellent communication skills both verbal and written required.
  • Completion of an accredited Medical Assisting program required.
  • Community Health Worker Certification preferred.
  • Able to work flexible hours, including early mornings, evenings, late nights, and weekends as needed.
