Position Engagement Specialist  
Description

New program seeking an Engagement Specialist to expand our capacity to serve individuals who present as walk-ins or new patients in need of linkage to services and treatment. This role is responsible for conducting brief assessments, building initial rapport, and linking patients with the appropriate services and support systems within our network. The ideal candidate is a proactive communicator with a strong sense of urgency and a commitment to health equity.

Qualified applicant will work out of our St. Clairsville and Martins Ferry offices.

Responsibilities:

  • Conduct rapid assessments via phone, in-person, or virtual platforms to identify patient needs and eligibility.
  • Provide immediate linkage and warm handoffs to internal and external services.
  • Maintain accurate and timely documentation in electronic health records.
  • Collaborate with clinical and care coordination teams to ensure continuity of care.
  • Identify barriers to care and work creatively to address them.
  • Follow up with patients to confirm engagement and address further needs.
  • Contribute to outreach and engagement strategies that improve patient access and treatment.
 
Position Requirements
  • Minimum of AA/BA required; LSW/LPC, CCDCII or II strongly preferred.
  • Experience with SPMI and/or SUD population preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Crisis intervention and de-escalation skills.
  • Knowledge of community resources and healthcare services.
  • Valid driver's license and insurance.
