Position Case Manager  
Description

This position provides case management services to promote recovery and wellness in adults with severe mental illness. Our Case Managers work as an integral part of an inter-disciplinary team to provide community-based supports, including resource development, promoting good health, crisis intervention and treatment planning for successful goal attainment.

We are a healthcare facility and as such we do require that our new employees are vaccinated (both COVID and FLU). Additionally, to promote a healthier workforce, we only hire non-smokers.

Position Requirements

Minimum of AA/BA required; LSW/LPC, CCDCII or II strongly preferred. Experience with this population preferred.
