The Clinic Care Manager offers proactive, integrated care coordination and care planning for improved care quality, access to services and resources, health and functioning of the patient, and quality of life. Care Managers provide for their patients by matching patient needs with appropriate services.   Position Requirements Must have an AA/BA or Community Health Worker certification. Qualified applicant must be willing to transport patients in their personal vehicle.
