Position Practice Transformation Advisor (Per Diem)

Description

The Healthcare Association of New York State (HANYS) is seeking a Practice Transformation Advisor within DataGen its for-profit subsidiary. Reporting to the Senior Practice Transformation Advisor, this per diem position is responsible for guiding and advising ambulatory primary and specialty care practice engagements in support of NCQA recognition programs (e.g., Patient Centered Medical Home, Patient-Centered Specialty Practice, and Health Equity) based on a defined Scope of Work and using DataGen's methodology and materials.

A fully remote work environment would be considered within New York State. Other states may be considered. The salary for this position is between $50.00/hr. and $57.00/hr.

Key responsibilities include but are not limited to:

  • Project Management - Ensure development of appropriate project plan templates per client Scope of Work. Maintain project plans, participate in and/or facilitate work team meetings to ensure on time completion of required tasks, identify and recommend risk mitigation strategies, communicate with site/system transformation team (s) on project progress. Provide support and resources as needed to facilitate their progress. Work with Sr. Practice Transformation Advisor to review project status and escalate barriers to progress.
  • Practice Transformation - Develop and apply change management plans to create awareness, build desire, establish knowledge and ability while reinforcing cultural and operational changes within site/system. Topics may include communication, training, leadership, job roles, performance management, workflow, and process efficiencies, etc.
  • Subject Matter Expert - Apply knowledge of NCQA PCMH Standards, NCQA Patient-Centered Specialty Practice (PCSP) Standards and Health Equity standards and processes for NCQA's recognition/sustained recognition.
  • Workflow Redesign - Facilitate the application of PDSA (Plan-Do-Study-Act) cycles and quality improvement principles such as lean six sigma to remove waste from existing workflows and improve practice operational flow efficiencies whether in an existing Electronic Medical Record (EMR) environment. Ability to understand the processes and workflows in the primary care setting and propose change based on experience and best practice.
  • Relationship Management - Remotely build and maintain client relationships with system administration, site staff members and physicians; ability to assess and understand client needs across and within sites/system, provide timely responses and communication. Ability to modify approach based on client culture and team structure.
  • Content Adaptation - Adapt as necessary DataGen's methodology and materials to the particulars of the medical practice and Statement of Work.
  • Training and Facilitation:
    • Effectively communicate verbally and in writing to present ideas and concepts in a way that fosters learning, collaboration, and transference of knowledge/skill into daily habit
    • Ability to assess various participants preferred learning styles. Engage clients using a variety of facilitation methods.
    • Effectively use DataGen's portfolio of education, materials, and tools for the practice sites.

  • Assure responsible and appropriate use of resources and time according to client specific Scope of Work, including timely intervention and the use of site visits, telephone contacts, and web-based capabilities.
  • Maintain required NCQA certified content expert (CCE) certifications.
  • Assist with sales calls to prospective customers.
  • Assist with participation in vendor exhibits at selected trade conferences.

Qualifications:

  • Bachelor's degree with at least four (4) years of ambulatory care experience. An equivalent combination of education and experience sufficient to satisfactorily perform the duties of the job may be substituted.
  • Experience with Patient-Centered Medical Home principles and models with a successful NCQA PCMH transformation track record.
  • Efficiently and effectively work in a self-managed home office setting.
  • NCQA Certified Content Expert (CCE) is required. If not certified as an NCQA PCMH CCE at the start of employment, certification must be obtained within six (6) months of hire.
  • Excellent project management, coaching, facilitation, collaboration, and communication skills required.
  • Ability to work independently as well as build consensus within group setting with various levels of staff and management.
  • Electronic Medical Record (EMR) and population health management experience is required.
  • Ability to travel on a limited basis, with potential over nights as required by client Scope of Work.
  • Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint).
