Position Medicare Outreach Enroller  
Division Nassau Suffolk Hospital Council  
Description

The Nassau-Suffolk Hospital Council based in Hauppauge, NY, is seeking a full-time Medicare Outreach Assistor and Health Insurance Enroller. Reporting to the Director, Health Insurance Programs, the position holder will be responsible for providing education outreach to DOH-identified Medicaid recipients who appear eligible for Medicare but are not yet enrolled. The position holder will provide information and education that will encourage them to enroll in Medicare so they can retain current Medicaid coverage and assist recipients initiate their Medicare applications through their local social security office. The position holder will also provide Medicaid enrollment assistance by phone or in-person to individuals that have lost their Medicaid coverage and need to re-enroll.

This is a grant-funded position, with the possibility of renewal yearly. The salary range for this position is between $53,000.00 and $59,000.00 annually,

Key responsibilities include but are not limited to:

  • Upon hire attend a mandatory company-paid 15-hour online certification training with the NY State Department of Health; complete course requirements and pass all certification tests to become a registered and certified CAC (Consumer Assistance Counselor) with the NY State of Health Marketplace.
  • Maintain certification as a NY State of Health CAC by attending all NYS mandatory webinar training sessions during the months of July through October every year; total of approximately 25 hours of company-paid online training each year; failure to complete training within the time-frame will result in loss of NYS certification; if there is a loss of certification, you will be required to re-take the mandatory, company-paid 15-hour certification training with NY State Department of Health and complete course requirements, which includes passing all certification tests.
  • Attend training provided by NY State Department of Health and Nassau-Suffolk Hospital Council to remain knowledgeable and current with all state and local rules and requirements of the following programs: Medicaid, Medicaid Buy-In for Working People with Disabilities (MBI-WPD), Medicaid Spend-down, Medicare Savings (MSP), and Medicaid for Institutional Care in a Nursing Home. Additional trainings regarding the Medicare Telephone Assistant program will also be provided, including an overview of the Medicare program.
  • Provide a minimum of three different outreach attempts to DOH-identified Medicare eligible recipients via phone calls (cold calling), encrypted emails, and written letters (average monthly list of 200 - 250 individuals to be completed each month). Help to provide education and understanding in their need to apply for Medicare to help keep their current Medicaid coverage in place. Assist clients in making appointments with their local social security office so they can apply for Medicare. Provide direct assistance either by phone or in-person to those who lost their Medicaid coverage by completing the paper Access NY Application or the online NY State of Health application for re-enrollment. Applicants reside throughout Nassau and Suffolk County and may also need help with enrollment in other government-sponsored programs such as Medicaid Buy-In for Working People with Disabilities (MBI-WPD), Medicaid Spend-down, Medicare Savings (MSP), and Medicaid for Institutional Care in a Nursing Home.
  • Assess the healthcare needs of the applicant and identify the program that best fits their needs; acquire the proper documentation that will best support their application. Conduct follow-up on all cases assigned to completion and track progress using state DOH reporting mechanisms and HANYS internal tracking system.
  • Provide enrollment assistance to applicants in the community as needed; work independently at these site locations; schedule follow-up appointments with applicants as needed; arrive on-time to all sites to set up and be ready to enroll at time session begins.
  • Instruct eligible applicants about Medicaid managed care and how to access benefits in a managed care environment; provide applicants with information about their right to appeal their eligibility decisions to the local departments of social services; provide them with documentation to help support their appeal.
  • Maintain data of all applications started in the internal data tracking system of HANYS/NSHC; monitor applications until completion; provide case notes on a regular basis to document all work performed on the case.
  • Monitor all applications started and submit within the required timeframes; late submissions will result in coverage being delayed and/or denied for an applicant.
  • Provide outreach as assigned to increase overall enrollment that will help all staff meet grant productivity deliverables (i.e., postcard campaigns, letter writing, staff occasional health fairs, document your outreach and assistance work using the CHA forms - 10 per month)
  • Maintain confidentiality of information contained in the application packet for government sponsored health insurance programs, as well as information contained in supporting documentation; secure all applications and documents in locked cabinets and safeguard keys; HIPAA and yearly internal information security training sessions must be attended, and protocols must be followed as specified by HANYS.

Qualifications

  • Bachelor's degree with prior experience in public health insurance preferred; High school diploma or GED with one year's experience in human services required.
  • Bi-lingual in English and Spanish preferred
  • Role requires some travel within Nassau and Suffolk Counties (up to 20% of work time may be spent outside of office); reliable transportation is required and the ability to maintain a flexible work schedule of 37.5 hours with occasional evenings and weekend hours; most time will be spent in the office on the phone.
  • Proficiency in Microsoft Office (Outlook, Word, Excel) and compliance with information security
  • Ability to process and synthesize information and make decisions.
  • Ability to communicate succinctly through writing and verbal communication.
  • Ability to handle adversity productively and be able to tolerate stressful situations.
  • Possess effective time management, organization, and planning skills; be able to meet all deadline requirements.
  • Be able to display teamwork, adaptability, reliability, integrity, accountability, and initiative.
