Case Manager plans for and monitors the following client outcome goals: reduced number of hospitalizations, increased number of days living in the community, increased use of community resources, and increased level of behavioral and physical health functioning. This person also maintains compliance with all applicable standards and must meet designated performance measures.   Position Requirements BA/AA required.   Location Southeast, Inc.
