Description

Job Title: Day Habilitation Direct Support Professional

Location: CP Rochester Winton Road Facility

Salary: $15.50 - $17.50 commensurate with experience

**This position is eligible for a $300 signing bonus! This bonus is earned and payable as follows: 25% ($75) in the pay period following the 3-month anniversary of your employment, 25% ($75) in the pay period following the 6-month anniversary of your employment, and 50% ($150) in the pay period following the one-year anniversary of your employment.**

Employment: This position may have a full-time or part time schedule. Days and hours of work are typically Monday-Friday between the hours of 8am and 4pm.  Full-Time is 35 hours per week and part time is 25 hours per week.

Summary/Objective:  Ensure safety and supervision of all individuals focusing on fulfilling the Agency’s Mission, Vision and Values and providing services which meet or exceed the goals of the individual, department and the agency. 

Requirements:

  1. High School Diploma or GED equivalent required.
  2. Experience working with people with developmental disabilities preferred.
  3. Valid and clean New York State Driver’s License.
  4. Demonstrated ability to effectively communicate orally and in writing.
  5. Ability to become CPR and first aid certified and successfully complete required training courses.

Essential Functions:

  1. Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources.
  2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
  3. Provides oversight to ensure safety and supervision of individuals.  May be required to work in the agency’s residential homes should there be an emergency, weather hazard, or other unforeseen occurrence.
  4. Provides assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to be more independent while addressing specific individualized outcomes as outlined on the staff action plans.
  5. Accompanies individuals to off-site locations/activities, helping them to access and utilize community resources (i.e. shopping, recreation, entertainment, etc).
  6. Assists individuals with planning and accessing activities of choice.
  7. Provides input regarding individual skill development, functional assessment and capabilities for team meetings/progress updates, etc.  Attends meetings as requested.
  8. Acts as a role model for program participants by interacting with others in a professional and respectful manner, dressing appropriately, and using good personal hygiene.
  9. Maintains necessary documentation as required and per regulations and agency policies.
  10. Communicates regularly with agency personnel to provide opportunities for exchange of information and discussion of concerns.  Provides feedback to Supervisor regarding individual and/or program concerns.
  11. Assists individuals in working toward achievement of their goals, while ensuring adherence to applicable Support Plans.
  12. Manages crisis situations per agency policies and procedures.
  13. Supports and encourages individuals to make as many decisions as possible, ranging from basic choices to long term goals.
  14. Provides emergency and first aid treatment, within their scope of training, as needed until relieved by other staff, a professional medical person, or other designated person.
  15. Attends all departmental or other related meetings/trainings. Participates and acts as a contributing member in the development and maintenance of person-centered Action plans.  Implements and monitors Action Plans on a daily basis.
  16. Participates in the team decision-making process and carries out decisions consistently per agency/department policy.
  17. Presents a positive and professional public image. Has a polite and professional interaction with individuals, families, staff, and members of the community.
  18. Attends all required training and any other training as assigned by supervisor.
  19. Participates in conferences, workshops and in-service training as needed or recommended to enhance job skills and personal growth and development.
  20. Adheres to agency’s policies and procedures as well as department guidelines.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit, stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 50 pounds.  See attached physical limitations.

Travel:  is primarily local during the business day (as needed), but employee may be expected to travel to a residential house in emergency situations.
