About the Organization CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations that have been serving communities in the Finger Lakes Region since 1969 and the Greater Rochester Area since 1946. Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.


Our staff consists of dedicated and knowledgeable professionals and clinicians with expertise in the areas of Special Education, Early Childhood Education, Physical, Occupational, Speech and Music Therapies, Psychology, Service Coordination, Nursing, Dental Services, and Developmental Disabilities.

Description

Job Title: Day Habilitation Coordinator

Location: CP Rochester – Winton Road Facility, Rochester, NY

Salary: $18.00 - $20.00 per hour commensurate with experience

Hours of Employment: This is a full-time position; 35 hours per week. Typical days and hours of work are Monday- Friday between the hours of 7:30am and 4pm with occasional early evening meetings.

Summary/Objective:  Develop and maintain individual charts and records in collaboration with the supervisory team. Completion of all applicable program documentation in accordance with program regulations and agency standards utilizing a person-centered approach and people first language.

Required Education and Experience:

  1. Bachelor’s degree in Education, Social Work, Human Services or related field required. Two years’ experience in working with people with developmental disabilities preferred.
  2. Valid NYS driver’s license and clean driving record required.
  3. Demonstrated organizational skills.
  4. Demonstrated interpersonal skills.
  5. Computer literacy skills and a demonstrated ability to adapt to computer software and programs.
  6. Demonstrated ability to effectively communicate orally and in writing.
  7. Demonstrated ability to exercise good judgment in planning, implementing, and evaluating work with individuals, customers, and employees.

Essential Functions:

  1. Develops meaningful partnerships with individuals, families, and the community to fulfill the individuals’ right to live productive and rewarding lives and to ensure all doors are open and opportunities are available.
  2. Provides support and encouragement to individuals of all ages and abilities to determine their pathway in life. 
  3. Advocates for people’s right to make meaningful choices, ranging from basic choices to long term goals.
  4. Attends Life Plan reviews.
  5. Reviews and updates Staff Action Plans and Individual Plans of Protection, to meet
  6. Regulatory body guidelines at least twice yearly and as needed or requested by those served.
  7. Trains program staff on daily documentation and individualized action plans.
  8. Evaluates the effectiveness of plans through monthly summary reports and functional assessments.
  9. Provides feedback to supervisory team based on observations of activities being offered, challenges and successes witnessed as individualized outcomes are addressed, and review of completed documentation.
  10.  Promotes feelings of self-respect and respect for others, as well as a sense of connection to the community.
  11. Completes and maintains all records in accordance with the standards of regulatory agencies governing the operation of CP Rochester.
  12. Meets with Supervisory team on a regular basis for exchange of information, brainstorming, and problem solving.
  13. Presents a positive and professional public image with individuals, families, staff, and members of the community.
  14.  Adheres to Agency & Program policy and procedures and serves as a role model for other program staff.
  15. Achieves program and department goals by performing other responsibilities as required.
  16. Maintains confidentiality of all protected health information in accordance with agency policy.
  17. Participates in quality and fiscal program audits, implementing recommendations, as needed.
  18. Performs all direct care services for residents when needed
  19. Attends all departmental or other related meetings/trainings as assigned by Supervisor.
  20. Participates in conferences and workshops recommended to enhance job skills and personal growth and development.
  21. Adheres to agency’s policies and procedures as well as department guidelines.

Work Environment: This job operates primarily at a busy site-based program. There may be noise, odors and exposure to cleaning and disinfecting solutions.

Physical Demands:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to sit, stand, walk, use hands to touch, handle or feel, and reach with hands and arms. Must be able to lift and/or move up to 50 pounds. 

Mental Requirements include: general intelligence, motor coordination skills, coordination of eyes, hands and feet, verbal intelligence and numerical intelligence.

Environmental conditions include: noise conditions and injury exposures.

Travel: Travel is primarily local during the typical business hours. The employee may be expected to travel for meetings and/or trainings or to a residential house to help cover during emergency closures.
