Description

Job Title: Behavior Support Specialist and Clinical Social Worker

Location: CP Rochester – Rochester, NY

Salary: $54,000.00 – 62,000.00 per year commensurate with experience

Employment: This is a full-time 40 hours per week position. Days and hours of work are Monday through Friday during business hours. Evening work may be required as job duties demand.

Summary/Objective: Evaluates the behavioral, cognitive, and emotional functioning of individuals with developmental disabilities. Offers recommendations and positive approaches to assist with behavior modification and development of new skills that will enhance quality of life for individuals. Functions as a member of a multi-disciplinary team providing counseling services in accordance with individual needs and treatment plans. The focus is on maximizing the potential of the individuals in all facets of their life and providing needed support to families and caregivers.

Required Education and Experience:

  1. Master’s Degree in the field of Psychology from an accredited program, with training in assessment techniques and behavioral program development preferred, Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW).
  2. Clinical experience with developmentally disabled individuals during education or work experiences in the field is preferred.
  3. Must have excellent oral and written communication skills.
  4. Must have demonstrated excellent listening skills.
  5. Must have a valid NYS Driver’s License.
  6. Computer proficiency.
  7. Must have demonstrated commitment to performing job duties and tasks at a high level of performance.

Essential Functions

  1. Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources.
  2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
  3. Provides behavioral diagnostics and behavior management programs and interventions.
  4. Develops a meaningful and realistic plan of service to maximize independence, productivity, and community integration.
  5. Develops and delivers services in areas including but not limited to: Behavior Management Planning, Family Support and Counseling and Functional Assessments.
  6. Recommends environmental modification for individuals.
  7. Trains staff in Behavior Management Plans, interventions, environmental modifications, crisis interventions, results of assessments, diagnosis, and prognosis.
  8. Documents all diagnostic assessment reports, management plans, and treatment notes accurately and in a timely manner and provides such information to treatment team as needed.
  9. Achieves program and department goals by performing other responsibilities as required.
  10. Consults with agency staff regarding the social and emotional needs of the individual and the family.
  11. Assists parents or caregivers in modifying attitudes and patterns of behavior by increasing their understanding of themselves, our individual, and their situation. Makes home visits as necessary.
  12. Develops psycho-social assessments/histories and/or social/emotional development assessments, which note areas of strength, areas of weakness, and coping mechanisms, of the individual and the family.
  13. As a member of a multi-disciplinary team, identifies emotional and social problems of individuals and families and identifies, mobilizes. Can advocate for individuals in regard to community resources and services to ensure the individual receives the maximum benefit of the services.
  14. Provides individual, family, and group counseling and appropriate social skill building activities as assigned.
  15. Attends meetings as assigned.
  16. If LMSW, attends monthly supervision with licensed Psychologist or LCSW.
  17. Assists in the periodic re-assessment of treatment plans in accordance with established procedures and regulatory agency requirements.
  18. Maintains documentation that meets the standards of the agency, regulatory Agencies and the standards of care.
  19. Adheres to agency, regulatory Agencies and professional policies, procedures, and standards of care.
  20. Achieves program and department goals by performing additional responsibilities as required.
  21. Participates in conferences, workshops and in-service training as needed or recommended to enhance job skills and personal growth and development.
  22. Adheres to agency’s policies and procedures as well as department guidelines.

Supervisory Responsibility: None.

Work Environment: This job operates in Residential settings as well as in an Outpatient Clinic setting.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit, stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 30 pounds and occasionally lift and/or move objects up to 50 pounds.

Travel: Travel is primarily local during the business day, but employee will be expected to work at any of our partner agencies. Some out-of-the-area and overnight travel may be expected.
