Position Overview
The Practice Transformation Specialist is a critical team member that supports Stony Brook Medicine's CIN clinical effectiveness and quality initiatives. The Practice Transformation Specialist serves as a key primary contact for practice groups, collaborating directly with the providers and office staff to identify opportunities for workflow improvement, integration of clinical and information technology resources to support quality initiatives, and to provide education and guidance around performance and patient outcome measures.
Responsibilities
• Serve as the primary contact and liaison for assigned provider practices.
• Engage and educate network providers, practice administrators, and clinical/clerical support staff to standardize clinical and operational workflow around ambulatory quality metrics
• Develop and lead efforts to increase provider adoption and use of various point-of-care solutions
• Use data analysis to identify opportunities for new clinical effectiveness initiatives
• Identify and promote opportunities for workflow standardization to improve patient and practice outcomes.
• Support CIN efforts to continuously evaluate, define, and implement network wide clinical initiatives and priorities
• Ensure clinical activities are appropriately documented and accurately reported for internal and external organizational requirements
. • Travel will be required to various clinical practice sites for meetings and training
