Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.

Description

Job Title: Van Driver and Direct Support Professional

Location: CP Rochester - Winton Road Site and Group Homes

Salary: $15.50 - $17.50 commensurate with experience

Employment:  Full time Day Shift Monday - Friday

Requirements:

  1. Must hold a currently valid NYS driver's license and have an acceptable driving record.
  2. High school diploma or equivalent and fluency in English required. Depending on assigned area, knowledge of Spanish would be helpful.
  3. Experience working with people with developmental disabilities preferred.
  4. Demonstrated ability to effectively communicate orally and in writing.
  5. Satisfactory completion of a company training program in passenger assistance techniques, sensitivity training, and map reading, or demonstrated competence based on previous employment experience.
  6. Satisfactory completion of CPR, first-aid, and seizure training provided by a local emergency medical training facility or a certificate indicating prior successful completion of such training.

Summary/Objective:  The Van Driver has the responsibility of safely transporting vulnerable individuals to and from the program, ensuring safety of all individuals and that all traffic laws are followed. The Direct Support Professional insures safety and supervision of all individuals focusing on fulfilling the Agency’s Mission, Vision and Values and providing services which meet or exceed the goals of the individual, department and the agency. 

Essential Functions:

  1. Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources.
  2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
  3. Ensures wheelchairs and passengers are properly secured before starting van. Depending on route, may pick up several individuals for a common destination.
  4. On arrival at program, assists individuals in exiting the van and reaching the proper program or treatment areas.
  5. Checks the van to ensure that no personal items are left behind.
  6. Provides first-aid or CPR if required during individual transport. Immediately notifies emergency staff and Director if skilled assistance is required.
  7. Communicates effectively with people receiving service and support, families, and staff.
  8. Immediately reports traffic accidents to Director, requesting police or ambulance service as needed. Assists injured persons from first-aid kit until these services arrive. Completes Agency accident reports.
  9. Tracks required maintenance of Agency vehicles and brings to the attention of Program Director, such as, but not limited to inspections, oil changes and routine maintenance.
  10. Provides oversight to ensure safety and supervision of individuals.  May be required to work in the agency’s residential homes should there be an emergency, weather hazard, or other unforeseen occurrence.
  11. Provides assistance, training and supervision with various activities of daily living, helping each individual to improve their ability to be more independent while addressing specific individualized outcomes as outlined on the staff action plans.
  12. Accompanies individuals to off-site locations/activities, helping them to access and utilize community resources (i.e. shopping, recreation, entertainment, etc.).
  13. Assists individuals with planning and accessing activities of choice.
  14. Provides input regarding individual skill development, functional assessment and capabilities for team meetings/progress updates, etc.  Attends meetings as requested.
  15. Acts as a role model for program participants by interacting with others in a professional and respectful manner, dressing appropriately, and using good personal hygiene.
  16. Maintains necessary documentation as required and per regulations and agency policies.
  17. Communicates regularly with agency personnel to provide opportunities for exchange of information and discussion of concerns.  Provides feedback to Supervisor regarding individual and/or program concerns.
  18. Assists individuals in working toward achievement of their goals, while ensuring adherence to applicable Support Plans.
  19. Manages crisis situations per agency policies and procedures.
  20. Supports and encourages individuals to make as many decisions as possible, ranging from basic choices to long term goals.
  21. Provides emergency and first aid treatment, within their scope of training, as needed until relieved by other staff, a professional medical person, or other designated person.
  22. Attends all departmental or other related meetings/trainings. Participates and acts as a contributing member in the development and maintenance of person-centered Action plans.  Implements and monitors Action Plans on a daily basis.
  23. Participates in the team decision-making process and carries out decisions consistently per agency/department policy.
  24. Presents a positive and professional public image. Has a polite and professional interaction with individuals, families, staff, and members of the community.
  25. Attends all required training and any other training as assigned by supervisor.
  26. Participates in conferences, workshops and in-service training as needed or recommended to enhance job skills and personal growth and development.
  27. Adheres to agency’s policies and procedures as well as department guidelines.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to sit, stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move objects up to 50 pounds.  See physical limitations below:

  • Ability to lift, push, pull, and position individuals frequently.
  • Ability to perform considerable reaching, stooping, and bending.
  • Hearing and vision abilities within normal range (corrected) to drive safely and to observe and communicate with individuals.
  • Need to handle emergency or crisis situations.
  • Exposure to a variety of weather conditions.
  • Occasional exposure to the risk of bloodborne diseases.
  • Must be able to bend, kneel, and move to secure wheelchair restraints for transportation as well as seat belts and shoulder straps and/or harnesses.

Travel:  Primarily local during the business day, but employee may be expected to work at any of our partner agencies.  Some out-of-the-area and overnight travel may be expected.
