Description

This is a full-time position. The CSP  Case Manager is responsible for linking individuals with mental illness to services in the community, and monitoring those services so that individuals obtain the greatest benefit towards independence and recovery.

Job Related Duties:

Subscribe to and practice therapeutic philosophies that promote well being and growth, allowing an individual to develop those skills with enable recovery

Conduct in-depth assessments of program participants through interviews, collateral contacts and other means of investigation with 30 days of admittance to the program

Complete an in-depth holistic plan of service in partnership with program participant that supports positive community living within 30 days of admittance to the program.

Coordinate services designated in the case plan including but not limited to: making phone calls to service providers, assisting with application completion, transporting program participants to appointments, assisting in securing safe, affordable housing and acting as liaison/advocate when necessary.

Monitor services being provided as an on-going assessment through face-to-face contacts, phone calls and written communications with program participants and collaterals to ensure that client’s needs are being met. Intervene when necessary.

Update case plans on an annual basis and/or as needed based on on-going assessment and monitoring. Document any changes through written addendums to the case plan. Complete required documentation within time frames set by CSP standards.

Develop discharge planning in partnership with program participant.

Record all contacts with program participant, family members and other professional’s contacts in a computerized database in appropriate format within 48 hours following Medicaid Standards.

Respond to crisis situations per certification regulations, policies and procedures

Provide recommendations, documentation and testimony regarding client’s mental health status in legal proceedings as needed.

Liaison with criminal justice system for program participants who are incarcerated.

Prepare and present oral and written reports as needed.

Participate in staffing of clients cases with agency staff and collateral contacts.

Participant inn night and weekend on-call rotation.

Other clerical, inventory, transportation and or other duties as assigned or deemed necessary by Director of CSP Case Management Services.

Ability to respond to the diversified needs of the client’s.

Requirements:

This position requires a Bachelor/Masters  Degree in a Human Services related field

A valid driver’s license and access to a vehicle are also required.

Other Requirements

This position may require a Caregiver Background Check subject to Wisconsin law. There may be no conviction record or pending charges relating to the care of adults or minors or the funds of adults or minors.

A significant portion of this job requires PC and/or desk work that involves continuous use of fingers, hands, wrists, and sitting for periods of time. The employee is required to talk, see and hear in order to meet with program participants within their own environment. The employee may occasionally lift and/or move up to 10 pounds.

The employee will encounter normal office working conditions and equipment. Essential job duties require the employee to travel to client residences and program related meetings.

 
Position Requirements

REQUIREMENTS:

1.  A Bachelor’s Degree in the human service or criminal justice field.

2. .At least two years of experience providing community based Case Management serving one or more of the following populations:

a. Individuals who have committed sexual offenses

b. Individuals with serious and persistent mental illness

c. Individuals who have a developmental disability d. Individuals who have committed violent crimes

3.  In accordance with the Department of Health Services, the SR Case manager must pass a WI Caregiver Background Check upon hire and annually thereafter

4.  Possess a valid WI Driver’s license and have a driving record clear of moving violations and at-fault accidents within the past 2 years
