The Position:

As the Behavioral Health Case Manager, you will have an opportunity to prevent homelessness by keeping individuals housed and living independently. You will be part of a unique team supporting residents living with mental illness who are at high risk of losing their residence.

 

The Case Manager’s primary responsibilities is to actively engage individuals experiencing mental illness and substance abuse providing case management services that will ensure housing stability, self-sufficiency and community integration. As a case manager you will be locating, procuring, coordinating, and monitoring the delivery of necessary services for the individual. You will be working with individuals to build skills that promote their mental health (these include social skills, coping skills, anger management, sobriety, and other life skills). You will be networking with agency and outside service providers, ensuring client access to appropriate resources. You will also develop and maintain a complete, accurate, and current client file with all required documents and data; ensure all paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases.

 

The ideal candidate should have knowledge of substance abuse and behavioral health disorders; good knowledge of available community resources, such as medical, legal, mental health and social services; skill in engaging a variety of consumers and in communicating clearly and effectively, both orally and in writing with consumers; ability to evaluate consumer needs and formulate goals to resolve social and emotional problems.

 
Position Requirements

Employment Standards:

  • Bachelor's Degree in psychology, social services, public health, guidance, or a related field required.
  • At least three years of experience in the provision of mental health and/or substance abuse treatment services, including some experience at the level of Caseworker; or any equivalent combination of experience and training which provides the required knowledge, skills and abilities.
  • Bilingual (English/Spanish) required.
  • MSW or AMSW with two years of experience in the provision of mental health and /or substance abuse treatment services to seriously mentally ill adults preferred.
  • One year of experience that includes working with clients in crisis, engaging individuals and their families in recovery-oriented treatment preferred.
  • Excellent clinical, interpersonal, analytical, computer, written, and verbal communication skills required.
  • Professional attitude, enjoyment of teamwork, and ability to maintain confidentiality a must.
  • Valid CA driver license and proof of insurance required.

 

Classification, Hours and Pay Rate:

  • This position is full-time, non-exempt, with benefits. (Benefits include: medical, dental, vision, life insurance, and 403(b) retirement plan. Generous holiday, vacation, and sick leave available.)
  • Pay is dependent on experience.
