With the belief that all people have the capacity to grow and change, we provide our services to people of all ages, cultures, races, religious preferences, genders, and sexual orientations in order to enhance wellness and recovery, thereby improving families, workplaces, and communities.   Position LPN   Description Full-time position to work in our primary care clinic to coordinate and provide care given to patients.  This person may call-in prescription orders, monitor medication and supplies, distribute medications, ensure proper linkages to resources, schedule, room patients and perform patient checks, provide patient education, and set up examination rooms.  The person in this position is responsible for following all infection control procedures. The person works within a team(s) supporting the patient and other staff in a Patient Centered Medical Home framework.   Position Requirements Ohio LPN or certified MA required.   As a healthcare provider, Southeast Healthcare is a non-smoking employer.  Staff members are required to be vaccinated for the flu and COVID.  Religious and medical exemptions may be requested during the hiring process.
