We believe in second chances, trauma-informed care, and person-centered planning. We are seeking a compassionate and organized Case Manager to support adults living with serious mental illness who also have a history of criminal justice involvement. Responsibilities: Provide community-based services to support patients in managing severe and persistent mental illness. Develop and connect patients to resources that aid in their recovery, such as housing, healthcare, and employment. Advocate for clients' access to benefits, treatment, housing, and employment opportunities. Promote and encourage healthy lifestyle choices, focusing on both physical and mental health. Offer crisis intervention in emergencies to de-escalate situations and ensure patient safety. Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet specific patient goals. Monitor patient progress and adjust treatment plans as necessary to ensure successful goal achievement. Experience working with adults with mental illness and/or criminal justice involvement required. Strong interpersonal and communication skills. Ability to work effectively as part of an interdisciplinary team. Crisis intervention and de-escalation skills. Knowledge of community resources and healthcare services. Experience with trauma-informed care and harm reduction preferred. Familiarity with HIPAA, Medicaid documentation, and local behavioral health regulations preferred.
