In this role, there is an emphasis on creating interventions that are strength-based and focused on promoting recovery, symptom reduction, increased coping skills, and achievement of the highest level of functioning for the patient in the community. Responsibilities: Provide community-based services to support patients in managing severe and persistent mental illness. Develop and connect patients to resources that aid in their recovery, such as housing, healthcare, and employment. Advocate for patients’ rights and needs. Promote and encourage healthy lifestyle choices, focusing on both physical and mental health. Offer crisis intervention in emergencies to de-escalate situations and ensure patient safety. Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet specific patient goals. Monitor patient progress and adjust treatment plans as necessary to ensure successful goal achievement. Experience with SPMI and/or SUD population preferred. Strong interpersonal and communication skills. Ability to work effectively as part of an interdisciplinary team. Crisis intervention and de-escalation skills. Knowledge of community resources and healthcare services. Valid driver's license and insurance.
