The Position

The PEARLS (Program to Encourage Active Rewarding Lives) Coach’s primary responsibility is to deliver PEARLS Program core services to the identified target populations and maintain fidelity to the program model. The PEARLS program is designed to reduce depression in community-based older adults aged 60 and older. Key activities for the PEARLS Coach include participant recruitment, assessment, and delivery of community-based direct services and interventions. Other key functions include timely completion of all documentation, compliance with data collection and submission requirements, and active participation in weekly clinical and programmatic supervision. Effective coordination with both internal and external providers and other PEARLS Team members is expected. This position reports to the PEARLS Program Manager.

Sample of Duties and Responsibilities

  1. Complete all program activities maintaining fidelity to the PEARLS model.
  2. Utilize assessment skills and screening tools to determine appropriate services.
  3. Exhibit a working knowledge of community resources and provide appropriate referral to support access to social, educational, and health services.
  4. Recruit new participants, assess new and enrolled participants according to program guidelines, develop client plans of care including goals and objectives, and regularly assesses progress toward goals.
  5. Conduct program outreach and represent FSA at collaborative meetings and community events.
  6. Provide strength-based, culturally responsive intervention with clients using the PEARLS problem-solving treatment approach.
  7. Meet deadlines, follow through on commitments, and complete all administrative and clinical documentation and data collection activities.
  8. Utilize databases to monitor and reflect program contacts, activities, and outcomes. Ensure client records and documentation are accurate, up-to-date, and confidential. Complete closing documentation including referrals to other services or crisis support services as needed.
  9. Provide interpretation and or translation support when appropriate.
  10. Proactively communicate and collaborate with clients and PEARLS team members. Maintain regular communication with the Agency and partners. This includes but is not limited to authorization for services, case consultation, daily checking of agency email and responding as needed, and attending as-needed individual supervision meetings and all required meetings and training.
  11. Attend weekly programmatic and clinical supervision meetings to report on cases, program requirements, and service delivery needs. 
  12. Meet the policy and procedure requirements of the agency and funders. See addendum. 
  13. Participate in all required agency and/or contractually mandated trainings. 
  14. Attend meetings with community partners to coordinate services. 
  15. Performs other duties as assigned
 
Position Requirements

Employment Standards:

  1. Bachelor’s degree in social work, gerontology, psychology, sociology or related field is preferred; OR 3 or more years’ experience working with older adults and/or providing case management services.
  2. A minimum of one year work experience in a community setting required.
  3. Bilingual in English and Spanish preferred.
  4. Experience working with at-risk seniors desirable.
  5. Exhibit cultural competency.
  6. An ability to relate warmly and professionally to older adults and community organizations is required.
  7. Computer and Internet literacy is required.
  8. Background check and TB clearance required.
  9. Valid automobile insurance and a valid California Driver License are required; reliable transportation is necessary to travel throughout Santa Barbara County.

Classification, Hours and Pay Rate:

  • This position is full-time (40 hours/week), non-exempt, with benefits, including: medical, dental, vision, life insurance, flexible spending accounts and 403(b) retirement plan. Generous vacation, holiday and sick leave also offered.
  • Pay range is from $25-27/hour depending on education and experience.
