Definition

The BH Care Coordinator is proactive in the responsibility of providing support to the daily operations of HealthWorks for Northern Virginia Behavioral Health program. The position responsibilities include a wide range of activities related to referrals, coordination, and care management of BH patients at HealthWorks. The position works closely with the BH Director in ensuring timely coordination of care, development of internal and external relationships, and growth of integrated care in the clinic. The BH Care Coordinator will also work closely with BH and Medical providers to ensure that patients are receiving necessary services through integration and appropriate use of resources. The BH Care Coordinator implements policies and procedures developed by the BH Director which ensure an accurate, timely, efficient, and transparent process for BH services. This entails processing referrals, providing follow-up with patient needs, determining the appropriate level of care and referral to outside resources. Data collection around current waitlist, processed and pending referrals is also a responsibility of this position.

Roles & Responsibilities

  • Ensures that all referrals sent to BH are processed within 24-48 hours.
  • Supports timely response to scheduling patients needing medication management with Psychiatric Nurse Practitioner (PNP).
  • Develops and maintains relationships with outside referral sources to ensure that the BH team has up-to-date information about who is accepting new patients, the process for admission, and who qualifies for which service. Ensuring that the team always has adequate up-to-date referral resource information readily available for patient and provider education.
  • Coordinates care with special partners such as CSB, MHDDSA, DSS, LAWS and others as needed after securing release of information.
  • Clear understanding of warm handoffs in the process of integrated care and completes warm handoffs for referrals.
  • Monitor specialty and primary care appointments for the patient and assist the patient in keeping those appointments (reminders, transportation arrangements, etc) as needed.
  • Assist patients with filling out applications as needed and coaching them on follow-up.
  • Collects and provides monthly reports of referral and schedule data.
  • Reviews all referral concerns and brings immediate concerns to BH Director for resolve.
 
Position Requirements

Qualifications & Education Requirement

  • Bachelor’s degree in related field or a minimum of 5 years of experience commensurate with the requirements of this role.
  • Strong time management skills; highly organized and detail oriented.
  • Fluency in Spanish required.
  • Strong verbal communication skills, with demonstrated ability to talk with diverse audiences.
  • Effective business writing skills.
  • Proficiency with Microsoft Office required (Word, Excel, Outlook, and PowerPoint).
  • Positive, mature, professional.

Desired Qualifications

  • Experience with integrated care models in FQHCs (Federally Qualified Health Center).
  • Knowledge of community health care systems and resources.
  • Experience with EMR.
  • Experience working in case management a plus.
  • Experience working with diverse populations a plus.

Knowledge, Skills, & Abilities

  • Assist with collection of data for outcomes and other studies as requested.
  • Assist with furthering integration of BH with Medical and Dental services within Healthworks.
  • Assist with improving productivity for the BH Team.
  • Serve as a link between HW providers, patients, and BH as needed.
  • Maintain compliance with workflows, policies, and procedures.
  • Other duties as assigned by the Director of Behavioral Health.
  • Demonstrate excellent internal and external customer service skills.
  • Employee must demonstrate an understanding of the intent, and follow standards, guidelines and protocols related to achieving and maintaining certifications by outside agencies.
  • Must possess the ability to follow supervisor’s instructions and maintain regular attendance.
  • Management may modify, add or remove essential job functions as necessary, or as changing organizational needs require.
  • Maintains respect for confidential information gained through employment at Healthworks, unless legally authorized or required to divulge such information.
  • Demonstrates ability to be flexible and highly reliable.
  • Demonstrates strong administrative and customer service skills to include prioritizing and executing a variety of tasks simultaneously; being self-motivated; ability to maintain cooperative relationships with patients, families, providers, staff and other customers/vendors.
