Case Manager - Mobile Crisis

Wyandot BHN is seeking a dedicated full-time Mobile Crisis Case Manager to join our Mobile Crisis Response team! Provide short-term community-based outreaching and engagement services to adult consumers with a mental illness, co-occurring substance use disorder or are experiencing a crisis in the community.  Once the immediate crisis is resolved and/or the person has established care with other service providers, the outreach specialist will provide a “warm hand off” to longer term service providers to ensure continuity of care. This position reports directly to the manager of the Mobile Crisis Response team. Mobile Crisis Case Managers will be responsible for providing services to both adults and youth clients.

 

Essential Functions:

  • Works with team members, community partners, and service organizations to provide direct service to adults who are experiencing a mental health crisis in the community. Direct services may include, but are not limited to: crisis intervention, assessment, short term crisis counseling, assist with connection to available resources, housing, employment and system navigation skills.
  • Utilize intentional outreach strategies throughout the community to connect individuals to services by reducing external barriers.
  • Follow-up with consumers upon discharge from the hospital or crisis stabilization centers.
  • Monitors program data and develops monthly reports.
  • Completes timely and accurate documentation of services in electronic medical record within 48 hours of encounter with client.
  • Actively participates in the research, development and implementation of emerging multi-system models and practices.
  • Coordinates community / mobile emergency responses in conjunction with the Crisis & Access Services team and Mobile Crisis Response team as needed.
  • Coordinates with 24/7 crisis line contractor as needed.
  • Confers with various Center staff to ensure optimum consumer care.
  • Models and inspires flexibility and creativity in the delivery of clinical services.
  • Assures staff and program compliance with Medicaid, State licensing regulations, and other contractual standards.
  • Respond to individuals during crisis situations as needed. 
  • Follow-up on referrals from family, friends, co-workers, and other professionals. 
  • Help individuals determine eligibility for Medicaid, Medicare, etc. 

Qualifications:

Education:      

•           Bachelor’s degree from an accredited University, with preferred experience in the human service and/or criminal justice fields.  Equivalent experience may be substituted in lieu of degree. 

Experience:

Preferred experience in the mental health field, specifically with community based and/or outreach work

Skills & Abilities:

  • Excellent interpersonal and communication skills, including both oral and written.
  • Ability to establish effective working relationships with both Center staff and various community agencies. 
  • Ability to work autonomously in the community and be proactive in outreach
  • Ability to facilitate planning meetings with individuals from multiple systems. 
  • Proficient in use of technology, including Microsoft Office. 
  • Provide and maintain dependable transportation and auto insurance. 
  • Ability to build and maintain close working relationships with clients and community partners. 
  • Complies with all relevant Center policies and procedures. 
  • Performs other related duties as deemed appropriate by immediate supervisor or designate. 
  • Ability to recognize and be sensitive to cultural and ethnic differences. 
  • Must be able to safely perform essential job functions, with or without reasonable accommodations. 
  • Clearance through the Kansas Child Abuse/Neglect Register and criminal background check.
