Position Case Manager - Franklin Station  
Description

Southeast Healthcare is looking for a PATH Case Manager to provide support to individuals who are experiencing homelessness. In this dynamic and rewarding role, there is an emphasis on creating interventions that are strength-based and focused on proving outreach and engagement to some of the community’s most vulnerable population.

 

 

 
Position Requirements

Minimum of AAS/BA required.

As a healthcare provider, Southeast Healthcare is a non-smoking employer.  Staff members are required to be vaccinated for the flu and COVID.  Religious and medical exemptions may be requested during the hiring process.
