Position Transition Youth Specialist  
Description

Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a qualified individual to provide community-based services with transition age youth 14 - 18 years old. Our programs are designed to assist participants with developing and strengthening Independent Living Skills, as well as the skills needed to obtain and maintain employment.  Duties include initial and ongoing evaluation of the participant’s independent living knowledge, vocational strengths, and interests; Developing plans to help participants obtain their identified goals including education, vocation, obtaining and maintaining employment; Collaborating with other service providers and systems to assure coordination of care; Developing resources and learning opportunities to assist participants with gaining independent living skills and becoming self-sufficient. 

Salary range based on education and related work experience = $35,000 - $42,660.

 
Position Requirements

Successful candidate will have an understanding of the developmental and behavioral challenges faced by transition age youth and must be willing to transport youth in personal vehicle as needed.

BA/AA degree required.
