Richmond Behavioral Health has a full-time position available for a Care Coordinator in our Medical Services unit to perform intermediate professional work assessing client needs, developing, implementing, and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned. Work is performed under the limited supervision of the Program Manager.
Essential Functions:
Monitors healthcare utilization of agency clients to include hospital discharges and emergency room visits in order to support the psychiatric providers in linking clients to appropriate care and/or developing and updating their treatment plan.
Collaborates with client and team members to support care plan and deliver services that aim to improve health outcomes; collaborates with various health care providers to coordinate care.
Reception of information on clients from local hospitals, court, school, and/or jail systems who are currently opened to psychiatric services.
Assist the psychiatric providers in developing an individual service plan of care and services appropriate to the evaluation with the client/family consensus; reviews and updates service plans and ensures forms, annual documents, and documentation in the electronic health record (EHR) are accurate, up to date and signed.
Assist in evaluating the client’s level of care. If needs increase, will assist in referring the case back to Rapid Access for case management.
Assists the front desk staff in mediating communication between the front desk, the prescribers, and the psych nurses.
Complete designated tasks within the client check-in and discharge from appointments process. Communicating with and guiding clients through navigating the appointment process including but not limited to the front desk, psychiatric prescriber, nurse, and primary care clinic.
Assisting with ensuring time-sensitive emergency and/or incident procedures are followed by involved staff members.
Coordinates client services and treatment with multiple service providers and agencies.
Evaluates the quality of services provided and changes in client’s condition and counsels clients as necessary; compiles and analyzes data; identifies and works to resolve problems.
Provides motivational counseling to clients to facilitate/improve engagement in services that support recovery.
Serves as liaison to public agencies and provides information regarding Authority programs and services.
Attends inter-disciplinary team meetings to discuss decisions for client plan of care and assists with discharge planning.
Provides crisis intervention and prevention services.
Prepares a variety of reports; prepares and maintains client charts and other records.
Attends meetings, staffings and conferences as they relate to client, staff, and program needs.
Assists in curriculum development and resource tools.
Other duties as assigned.
