Description

JOB SUMMARY:  Responsible for providing outreach to community partners and potential program participants, screening for all applicants, and case management services to enrolled households. 

ESSENTIAL FUNCTIONS:  

  1. Conduct outreach to community partners and potential program participants, includes membership in, attendance at, and meaningful participation in county CoC general assembly/ housing service meetings and committees and other resources serving program’s target population. 
  2. Provide prompt response and screening to all referrals in 24 hours of receipt.
  3. Complete intake assessment and document collection for all households presenting as eligible for and in need of services within 3 days of screening.
  4. Develop an individualized Housing Stabilization Plan (HSP) for each client that outlines the steps needed to prevent homelessness and/or to stabilize housing within 1 week of assessment. Update HSP as steps are completed or as barriers are encountered with next steps to overcome.
  5. Provide wrap around case management services including but not limited to education, regular contact, support and guidance throughout the diversion process.
  6. Offer low/no barrier access to service including traveling to consumers (and/or coordinating/ providing transportation to help clients travel to necessary appointments) and/or coordinate translation services or other accommodations for individuals who have limited English proficiency or other disabilities.
  7. Maintain client records in hard copy and electronic database and completes documentation according to program and agency accreditation standards.  This may include, but is not limited to 24 hr turn-around of data entry, progress notes, activity reports, time sheets and logs.
  8. Coordinate and/or provide financial assistance to help individuals pay for expenses related to housing stabilization in collaboration with Program Director and Business Manager.
  9. Engage in regular case coordination with OHP including meeting and training attendance.
  10. Participate in annual Point in Time Count.
  11. Other duties as assigned.

 

 
Position Requirements

MINIMUM QUALIFICATIONS:  MSW preferred or Master’s degree in the behavioral sciences, human services or social services fields with specialized experience with housing services and/or children and families; or a Bachelor’s degree with 3+ years experience with housing services, related social services, and/or children and families; or HS Diploma with 5+ years of experience.

Skills to include case management, database entry and file management experience - bilingual Spanish preferred.

PREFERD QUALIFICATIONS: Bilingual Spanish-Speaking; lived experience with housing crisis

LICENSES/CERTIFICATIONS NEEDED:  Valid driver’s license
