MidMichigan Community Health Services is a federally qualified rural community health center (FQHC) based in Houghton Lake, Michigan. We provide primary and specialty health care for all, with a particular emphasis on access for the underserved.

We are headquartered in beautiful Houghton Lake, Michigan! Northern Michigan has an abundance of year-round activities for all. Our community offers great fishing, beautiful parks and beaches and is home to the largest inland lake in Michigan! Exploring Northern Michigan will provide the opportunity to explore wonderful parks, great public beaches which include swimming, playground equipment, barbecues and pavilions. From fishing to disc golf, our local parks are sure to provide fun for all. Additionally, the area provides plenty of hiking and nature trails, and over 200 miles of ORV trails.

This position is located in Coleman, MI Schools. The Village of Coleman is located in Midland County which has a wealth of recreation opportunities. Bicycle on the Pere Marquette Rail-Trail. Play soccer at the Midland Soccer Club where there's over 30 fields across 54 acres. A visit to the Chippewa Nature Center provides opportunities for hands-on learning at their Nature Discovery Area. And we're home to the Whiting Forest Canopy Walk... the longest in the nation!

Our organization was founded by concerned residents in the early 1970s to address the severe shortage of primary care in our area. We are still run by a local volunteer board of directors, helping to ensure that our services meet the community's needs.

SUMMARY

This position works as an LMSW or LPC Counselor to provide a full range of effective Behavioral Health services to adolescent patients ages 5-21. Participates in interdepartmental, intersystem and external committees, meetings and activities to promote the mission, values and business of MidMichigan Community Health Services. Works under the clinical supervision of the Director of Behavioral Health

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides counseling services to individuals, families and groups, based on individual treatment plans. Plans treatment based on the individual’s needs and resources as well as on clinic and community resources.
  • Maintains good communication and collaborative relationships with referring medical providers and other staff and recommends additional referrals for patients as needed. Coordinates with professionals involved in patient care to insure collaboration and continuity of services.
  • Maintains comprehensive and current knowledge of community resources and assists patients as needed in accessing these resources as appropriate. Is available for consultation with other clinic staff regarding community resources.
  • Provides crisis assessment and intervention as appropriate regarding degree of danger and immediacy of crisis situations. Designs and carries out appropriate interventions in crisis situations.
  • Carries out all duties with the highest professional standards, especially maintenance of patient confidentiality, as delineated in the Codes of Ethics appropriate to profession.
  • Represents MCHS to the community at large in a professional and helpful manner.
  • Maintains pertinent patient information in the medical record. Completes progress notes in a timely fashion.
  • Fulfills additional responsibilities as requested.
  • Keep up to date with new and revised state and federal regulations and statutes related to community health centers and patient care.
  • Actively participate in a variety of committees as assigned.

Position Requirements

MINIMUM QUALIFICATIONS/ABILITIES

  • Licensed Masters Social Worker, or Licensed Professional Counselor.
  • Completion of internship required by degree.
  • One year of supervised experience in counseling setting.
  • Current Michigan driver’s license.
  • Access to reliable transportation to travel between clinic sites.
  • Familiarity with electronic medical records or willingness to learn.
  • Ability to function effectively, independently, and efficiently in a stressful and dynamic work environment.

EDUCATION, EXPERIENCE, TRAINING AND SKILLS

  • Knowledge of the principles and practices of counseling.
  • Knowledge of current social and health problems related to psychological disorders and subsance use.
  • Knowledge of community resources and referral agencies.
  • Knowledge of group dynamics.
  • Skill of counseling.
  • Skill in interviewing and communicating to elicit information and cooperation from patients.
  • Skill in establishing and maintaining effective working relationships with patients, staff, and the public.
  • Ability to evaluate, diagnose and treat patients.
  • Computer skills sufficient to write reports and document patient progress using electronic medical records.
