Midlevel Clinical Care Coordinator

Position Midlevel Care Coordinator  
Description

Unique opportunity to provide evidence-based and outcome-focused care coordination for patients as part of a member of a motivated multidisciplinary care team! The Care Coordinator will assist team-based care for individuals to resolve or maintain mental, emotional, social, functional, and physical health. The Care Coordinator will receive training to offer professional consultation and education to team members and patients, as well as provide patients with advocacy, linkage, and care coordination with other health or social service agencies as needed.

Qualified applicant will provide behavioral and medical health screenings and interventions with a trauma-informed approach, display excellent engagement skills with patients and team members, exhibit high energy, flexibility, and adaptability to variable paced environments, be well organized and respond to care gaps for patients.

 
Position Requirements
  1. AA/BA or CHW required; LPN, RN, LSW or LPC strongly preferred.

    Valid Ohio Driver’s License, car insurance and willing to transport patients in own car required. Knowledge of community agencies and the skill to negotiate the system with the patient required.

  2. As a healthcare provider, Southeast Healthcare is a non-smoking employer.  Staff members are required to be vaccinated for the flu and COVID.  Religious and medical exemptions may be requested during the hiring process.
