KSHOP Liason 

Description

Essential Functions:

  • Provide community based intensive case management services to adult consumers with a mental illness and/or co-occurring substance use disorder who have released from a prison environment. These individuals are identified primarily through Parole and Department of Corrections.
  • Direct services may include, but are not limited to: crisis intervention, assessment, mobile outreach, travel with clients to appointments and court-hearings, assist with applications for benefits like Medicaid and disability, housing, employment, job development skills and system navigation skills.
  • Work with the consumer while still incarcerated to prepare him/her for release;
  • Establish a trusting relationship with consumers, maintaining hope, dignity, accountability and respect.
  • Assist with reentry by helping to establish a plan of action with specific strategies to be carried out by this position to ensure the consumer is connected to necessary mental health services, including medication management and other support to help keep housing.
  • Use motivational interviewing techniques to actively engage the consumer in ongoing services.
  • Identify strengths and needs of consumers and work to involve them in recovery oriented planning for desired services.
  • Work to fully integrate consumers into the community.
  • Participate in multi-disciplinary team meetings, pre-and post-release, to address barriers and maximize supports. This includes communicating with corrections staff (in facility and parole), housing provider/liaison, substance abuse treatment provider, care coordinator, and the court about the consumer’s progress, needs, treatment, etc.
  • Work closely with families of consumers to provide education and support.
  • Link the consumer to other resources (peer support, supported employment, outpatient services, and psychiatric medical services) and with community resources (educational, cultural, recreational and social activities).
  • Work closely with DCF, SSA, and other agencies as appropriate to secure financial support and other benefits.
  • Assist consumers in maintaining housing and promote stability through on-going support.
  • Travel to correctional facilities as needed and attend necessary trainings as required by KDOC.
  • Maintain a collaborative relationship with stakeholders, including the Department of Corrections, Parole, Court, and HRADAC-Heartland Regional Alcohol and Drug Assessment Center.
  • Familiarity with Crisis Intervention Team and de-escalation strategies recommended.
  • Assists with the development of treatment plans to help reduce/prevent hospitalizations with a goal of not re-entering the prison system.
  • Acts as a boundary spanner for criminal justice systems, mental health systems, and additional community based service organizations.
  • Monitor program data and develop monthly reports to track referrals.
  • Completes timely and accurate documentation of services in electronic medical record.
  • Actively participate in the research, development and implementation of emerging multi-system models and practices.
  • Coordinate with 24/7 crisis line contractor as needed.
  • Coordinate with various Center staff to ensure optimum consumer care.
  • Models and inspires flexibility and creativity in the delivery of case management services.
  • Prepare letters to the court and testify in court as needed.
  • Assure staff and program compliance with Medicaid, State licensing regulations, and other contractual standards.
  • Work with KDOC and parole staff to address system issues that create barriers to offenders receiving mental health care timely and effectively, engaging in strategic planning and implementation of identified solutions.
  • Participate in case staffing with staff from various community partners.
  • Serve as a point of contact at WCCBHI for parole and KDOC
  • Job Duties will include using own vehicle during the course of employment and will include transporting clients in own vehicle at times.

Education:

  • Must have a Bachelor degree from an accredited University, with preferred experience in the human service and/or criminal justice fields. Equivalent experience in lieu of a Bachelor’s degree will be considered.

Experience:

  • Excellent interpersonal and communication skills, including both oral and written. Experience and rapport development skills with people experiencing acute crisis. Ability to establish effective on-going professional working relationships with clients, internal partners, and various community partners. Computerized database, word processing, and spreadsheet experience required.

  • Exceptional verbal and written communications skills, including the ability to write and speak concisely, succinctly, and accurately in a style appropriate to the audience.
  • Must have excellent interpersonal skills and be at ease in conversing with constituents at all levels.
  • Must have the ability to be diplomatic and professional at all times, while exercising excellent judgment and discretion as appropriate.
  • Ability to build and maintain close working relationships with clients and community partners.
  • Must be comfortable working in an urban environment with individuals who have been convicted of felony crimes.
  • Persuasive, tactful and discreet with excellent follow-through essential.
  • Must have excellent organizational skills; be detail-oriented, able to effectively multi-task.
  • Demonstrated or proven ability to set priorities, attain deadlines, while balancing multiple projects.
  • Flexible and adaptable as demands and focus shift.
  • Must be self-directed, a self-starter, and have the ability to work under minimum supervision.
  • Must be tech-savvy and proficient with Microsoft Office.
  • Exceptional rapport building skills with persons in crisis.
  • Strong decision making skills in fast-paces environment.
  • Creative, innovative, and flexible approach to work functions.
  • Bilingual and bi-cultural preferred.
  • Ability to work with some flexibility in work hours.
  • Complies with all relevant Center policies and procedures.
  • Performs other related duties as deemed appropriate by immediate supervisor or designate.
  • Ability to recognize and be sensitive to cultural and ethnic differences.
  • 2-3 years experience in delivering case management services preferred.
  • Demonstrated ability to work with people who suffer from Severe and Persistent Mental Illness (SPMI)
  • Must be able to safely perform essential job functions, with or without reasonable accommodations.
  • Clearance through the Kansas Child Abuse/Neglect Register and criminal background check.
  • Ability to complete a KDOC for a background check to ensure the person is eligible to enter correctional facilities and parole offices, to perform duties.
  • Must have dependable transportation, a valid driver’s license and comprehensive collision insurance on vehicle.
