The Catalyst Case Manager will develop knowledge at the community level that is vital to addressing health disparities, and work to build individual and community health. The main function of this position includes identifying and implementing projects and activities that build stronger communities and individual health. The person in the position will work to form and support community-based partnerships, encouraging collaboration to support individual and community health, work within a plan to gather measurable outcomes, and create lasting relationships to combat health inequity. The Case Manger work on an individual level with residents who are Southeast Healthcare Patients and other residents who live within an apartment building and within a defined community neighborhood. Knowledge of mental health, substance use challenges, chronic health issues, nutrition are essential for the successful candidate. Community building skills and teamwork are required.   Position Requirements BA/AAS required.   Location Southeast, Inc.
