The Position

The Family Support Services Program is adding a new position! FSA contracts with Cottage Hospital and local health clinics to provide supportive services to families with young children ages 0-3. Parents and children who experience Adverse Childhood Experience (ACEs) are at heightened risk for toxic stress that interfere with wellness. The Wellness Navigator will work collaboratively with a Health Promotion team and will partner with families to further assess developmental and social and emotional health issues. Using the Protective Factors Framework, the Wellness Navigator will equip families with the skills and knowledge needed to overcome sources of stress, strengthen resilience, and lessen the impact of ACEs through trauma informed interventions and support.

The Wellness Navigator will be based out of FSA’s Lompoc office, and may also be located at health clinics in the community. This position will provide services as needed in the home, community, and health care setting.

Because working with those impacted by trauma can affect staff in various ways, FSA actively identifies and employs methods to support staff to embody a healthy and sustainable balance of care for both themselves and others.

 

Sample Job Duties and Responsibilities

  • Educate families on the impact of Adverse Childhood Experiences and use the Protective Factors Framework to guide appropriate trauma-informed interventions. 
  • Lead the process of engaging, interviewing, and assessing families with children ages 0-3 to decrease sources of stress and to promote resilience and family wellness.
  • Utilize a resiliency-oriented, strengths-based approach in delivering treatment services.
  • Collaborate with a multidisciplinary team and serve as a liaison between families, health care providers, and community-based organizations.
  • Support families through a trauma informed approach by providing brief, focused prevention services and ongoing care coordination.
  • Use assessment skills and screening tools in collaboration with families to promote protective factors and access community resources.
  • Ensure client records and documentation is accurate, up-to-date, and confidential.
  • Utilize databases to monitor and reflect program contacts, activities, and outcomes.
  • Attend supervision meetings to report on cases, program requirements, and service delivery needs.
  • Participate in all required agency and/or contractually mandated trainings.
  • Attend meetings with community partners to coordinate services and promote continuity of care.
 
Position Requirements

Employment Requirements:

  • Bachelor’s degree in Social Work, Early Childhood Education, Psychology, or related field is required.
  • Ability to speak, read, write, and understand English and Spanish required.
  • A minimum of one year experience working in health care or community settings providing services to children and/or families.
  • Ability to have a warm and empathetic interpersonal style to establish trusting relationships with children, parents, health professionals, and community organizations.
  • Must have the ability and desire to provide services that are relevant to the culture of the family.
  • Ability to work function effectively as a part of a multi-disciplinary team including primary care and behavioral health providers, patients, and family members.
  • Ability to assess and continually improve services delivered as an individual provider and as a part of a multi-disciplinary team.
  • This position requires the following vaccinations and Tests: MMR (Measles, Mumps, Rubella), Varicella, Tdap, Seasonal Flu Vaccine, TB Test. For more information, please refer to FSA’s vaccination policy.
  • Possess a valid California Driver’s License, proof of vehicle insurance, and reliable transportation.
  • Valid automobile insurance and a valid California Driver License are required.
  • Reliable transportation is necessary to travel throughout Santa Barbara County.

Classification, Hours and Pay Rate:

 

  • This position is full-time (40 hours/week), non-exempt, with benefits, including: medical, dental, vision, life insurance, flexible spending accounts and 403(b) retirement plan. Generous vacation, holiday, and sick leave also offered.
  • Starting pay for applicants is $22.00/hour.
