With the belief that all people have the capacity to grow and change, we provide our services to people of all ages, cultures, races, religious preferences, genders, and sexual orientations in order to enhance wellness and recovery, thereby improving families, workplaces, and communities.   Position Care Coordinator for Multi-System Adults   Description New funding for a full-time Care Coordinator to work with adults with mental illness involved in multisystems (criminal justice, homeless, hospitals, etc.). The Care Coordinator will provide outreach, foster relationships with systems for referrals, receive referrals of adults, create linkages to behavioral health providers, and plan for the use of funding to support recovery and decrease dependency on multiple systems. As a healthcare organization, we hire non-smokers.  COVID and flu vaccinations are required of staff members.  Medical and religious exemptions may be requested during the hiring process.   Position Requirements Qualified applicant will have a high level of skill in data management and reports, high accountability, experience with adults with mental illness, and expertise with Excel.
