DS Lead Support Coordinator-Adult  

Description

Richmond Behavioral Health is seeking full-time DS Lead Support Coordinators to join our Adult Developmental Services Team. The incumbent serves as a Developmental Services Lead Support Coordinator with primary responsibility for providing direct support coordination services and oversight to support coordinators for the provision of services to individuals with intellectual and/or developmental disabilities and their families. The incumbent performs difficult professional and administrative work coordinating case management activities, overseeing the work of staff within a program, ensuring the quality of program services, coordinating services with other divisions and community partners, maintaining records and files, and related work as apparent or assigned. Work is performed under the limited supervision of the assigned supervisor.

Working from a team approach and guided by person-centered thinking, the incumbent will provide assessment, planning, linking, monitoring, education and advocacy to adults and children who have an intellectual and/or developmental disability or children under the age of six who are at a development risk. The incumbent will ensure that needed services and supports are identified, delivered and documented according to the individual’s Person-Centered Plan. The incumbent will complete comprehensive documentation that meets regulatory, agency and reimbursement requirements. In carrying out the position requirements, the incumbent will maintain a positive working relationship with a variety of private providers, agencies, schools and professionals.

 

Essential Functions

  • Assess the individual and family needs by completing approved assessments, collecting/reviewing records and interviewing the individual, family/caregiver, and other providers; complete intakes if applicable.
  • Develop Individual Service Plans with the individual, family/caregiver and other providers as appropriate; reviews and updates service plans.
  • Monitors and evaluates service delivery in accordance with established guidelines and principles; enforces program policies and procedures.
  • Evaluates the quality of services provided and changes in individual’s condition and counsels individuals as necessary; evaluates individuals’ environment for safety, security, and negative factors.
  • Assists in the recruitment and selection of program personnel; assigns, helps train and inspect the work of staff; coaches, counsels and evaluates staff performance. Assist in orientating newly hired staff and facilitate shadowing opportunities.
  • Support supervisor to monitor and oversee the work of Support Coordinators within unit.
  • Monitors and audits clinical and medical records and practices for compliance with regulatory, reimbursement and agency standards; reviews documentation for accuracy.
  • Provides supervision and consultation during training, team meetings, quality assurance, and caseload assignment.
  • Provides crisis intervention and prevention services to individuals and families; provides consultation and education to staff and clients.
  • Report suspected cases of abuse, neglect and exploitation to the appropriate authorities;
  • Represent the agency in committee and community groups in terms of networking and linking individuals with needed services and supports.
  • Attends meetings, staffings and conferences as they relate to client, staff and program needs.
  • Perform other assigned duties consistent with established policies and procedures.

 

 

 
Position Requirements

Knowledge, Skills and Abilities

Knowledge of:

  • The definition, causes and program philosophy of intellectual/developmental disability.
  • Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention,discharge planning and service coordination;
  • Different types of assessments and their uses in program planning.
  • Individual rights.
  • Local community resources and service delivery systems, including support services, eligibility criteria and intake processes, termination criteria and procedures, and generic community resources.
  • Types of intellectual/developmental disability programs and services.
  • Effective oral, written, and interpersonal communication principles and techniques.
  • General principles of record documentation.
  • The service planning process and the major components of an Individual Support Plan.

 

Skilled in:

  • Interviewing.
  • Negotiating with individual consumers and service providers.
  • Observing, recording, and reporting behaviors.
  • Identifying and documenting an individuals need for resources, services, and other assistance.
  • Identifying services to meet the individual’s needs.
  • Coordinating the provision of services by diverse public and private providers.
  • Analyzing and planning for the service needs of individuals with developmental disabilities.
  • Formulating, writing, and implementing Individual Support Plans to promote goal attainment for individuals with intellectual/developmental disabilities.
  • Successfully using assessment tools.
  • Identifying community resources and organizations and coordinating resources and activities.

 

Abilities to:

  • Demonstrate a positive regard for individuals and their families (e.g. permitting risk taking,avoiding stereotypes of individuals with developmental disabilities, respecting individuals' and families' privacy, believing individuals can grow).
  • Be persistent and remain objective.
  • Work as a team member, maintaining effective inter-agency and intra-agency working relationships.
  • Work independently, performing position duties under general supervision.
  • Communicate effectively, verbally, and in writing.
  • Establish and maintain ongoing supportive relationships.

 

Education and Experience:

A minimum of a bachelor's degree in a human service field including, but not limited to, sociology, social work, special education, counseling, or psychology and considerable experience working with individuals with intellectual and/or developmental disabilities. An advanced degree in a related field fulfills the requirements if a qualified candidate does not have a bachelor’s degree in a related field area. Preferred 3 years of documented experience working directly with individuals who have intellectual and/or developmental disabilities.

Special Requirements

Valid driver's license in the Commonwealth of Virginia.

QDDP status upon hire.

Starting Salary Range $55,089.43, negotiable based on experience
