The Position

The Housing Authority (HA) Case Manager/Clinical Case Manager provides case management along with service and resource coordination with clients that are low-income and in crisis or emergency situations.  Family Service Agency contracts with the Housing Authority of the City of Santa Barbara (HACSB) to provide case management services to support the unique social service needs of residents of HACSB. The residents include families, adults, and seniors. The Case Manager/Clinical Case Manager will work collaboratively with clients, Housing Authority, and public and private agencies to guide households in overcoming obstacles to self-sufficiency.  Through advocacy, planning, coordinating, and linkage to community resources, the Case Manager/Clinical Case Manager will identify risks, promote strengths, and equip clients with the skills and knowledge needed to overcome sources of stress, strengthen resiliency, and health disparities.  Services are provided in community settings, FSA/HACSB offices, and in the home. This position reports to the Housing Authority Program Supervisor.

This job opening has one position available, and our desired candidate can be a Case Manager or Clinical Case Manager, depending on education and experience. The Clinical Case Manager will have an MA and will work at the Associate or Licensed level. The Case Manager will work at the BA level and is not on a clinical licensing path.

Sample Job Duties and Responsibilities

  1. Provide case management services, often face-to-face.
  2. Conduct clinical assessments evaluating the current status of clients’ mental, emotional, and behavioral health and considering relevant cultural issues and concerns. (Clinical Case Manager only.)
  3. Provide advocacy/assistance for residents in obtaining public benefits.
  4. Foster positive interagency relationships, and provide referrals for any indicated outside resources.
  5. Exhibit a working knowledge of community resources and provide appropriate referral to support access to social, educational, and health services.
  6. Support residents in accessing substance abuse counseling/recovery and/or medical/mental health services and provide coordination of behavioral health and clinical services.
  7. Provide and/or arrange crisis intervention services, mental health assessments and treatment planning. (Clinical Case Manager only.)
  8. Provide and/or arrange for residents to develop/strengthen independent living skills.
  9. Collaborate with Santa Barbara County Department of Behavioral Wellness to ensure access to treatment for qualifying residents. (Clinical Case Manager only.)
  10. Collaborate with HACSB staff and landlords to address/resolve lease compliance issues to promote housing retention.
 
Position Requirements

Employment Requirements:

  1. HA Case Manager: Bachelor’s degree required, with preference of major course work in social work, psychology or another closely related field.
  2. HA Clinical Case Manager: This position requires a registered AMFT, APCC, or ASW.
  3. Bilingual (English/Spanish) preferred.
  4. Ability to communicate ideas clearly and concisely, both verbally and in writing, is required.
  5. At least two (2) years of experience in social case work, eligibility determination, social service or community resource referral, or closely related work preferred.
  6. Minimum of two (2) years working with diverse communities, low-income clients, the homeless and formerly homeless, seniors, and the disabled preferred.
  7. Knowledge of general interviewing practices and/or techniques; knowledge of case management practices and techniques, including crisis prevention and intervention preferred.
  8. Excellent interpersonal, analytical, computer, written, and verbal communication skills required.
  9. Professional attitude, enjoyment of teamwork, and ability to maintain confidentiality a must.
  10. Reliable transportation, valid CA driver license, and proof of insurance required to travel throughout Santa Barbara.

Classification, Hours and Pay Rate:

  • This position is part-time, 20 hours/week. Limited benefits are offered, including: flexible spending accounts and 403(b) retirement plan. Pro-rated vacation, holiday, and sick leave also offered.
  • Pay range for the Case Manager position is dependent on experience and language ability: $22-$24/hour
  • Pay range for the Clinical Case Manager position is dependent on licensure level and language ability: $24-$30/hour
