Description of Duties:

Works as a member of the health care team under the supervision of the Director of Nursing. The duties of the Clinical Access Nurse include:

Patient Care

  • Acts as a telephone triage nurse by obtaining clinically appropriate data and symptoms from patient interviews.
  • Advises and instructs patients according to established protocols and/or orders from the provider.
  • Appropriately and accurately documents all patient care interactions in patient electronic medical record.
  • Assists with patient inquiries for appointment scheduling, and prior authorizations as needed.
  • Coordinates telephone requests to refill prescription drug medications in accordance with physician’s directions and protocols.
  • Conducts assessments of callers presenting symptoms.
  • Provides oversight and support to the non-clinical staff.
  • Possesses and maintains a thorough grasp of clinical knowledge pertaining to various disease states, medications, treatments, etc.
  • Ability to communicate in an effective professional manner, with various types of people, regardless of their age, background, skills and abilities.
  • Provides health and wellness education for all clients delivered in a culturally competent manner.
  • Demonstrates an appropriate knowledge of the meaning of informed consent and ensures that informed consent is obtained for procedures as appropriate per protocol.
  • Educates patients and family members on the patient’s diagnoses within scope of knowledge and authority.

Clinical Skills

  • Demonstrates ability to carry out skills appropriate to patient population served within scope of practice.
  • Demonstrates ability to use equipment according to nursing procedures and as used in the department.
  • Demonstrates ability to assist providers with treatments as appropriate.
  • Demonstrates ability to carry out procedures following established standards of care.
  • Demonstrates an appropriate understanding of medications and pharmaceutical aspects associated with drug considerations.
  • Serves as a mentor/example in quality patient care and adherence to clinical protocols for other clinical staff
  • Participates in Center projects, initiatives, and/or committees as requested.
  • Completes appropriate paperwork in a timely and legible manner.
  • Ensures that appropriate consent forms are signed and reviews forms associated with different programs to ensure that all paperwork is in order.
  • Is proficient in documentation and use of the EMR system to include eClinicalWorks (eCW) etc.
  • Demonstrates professional development and clinical competency.

Administrative Duties

  • Performs various duties to ensure quality patient care, excellent patient flow, and efficiency of operations with regard to telephone nursing duties.
  • Completes appropriate paperwork in a timely and legible manner.
  • Keeps professional licensures and certifications active, current status and renews timely.
 
Position Requirements

Qualifications: Current Commonwealth of Virginia Licensed Practical Nurse.English/Spanish fluency required. Minimum of two years clinical experience at an acute care or ambulatory setting preferred. Supervisory or leadership experience preferred. Community or public health experience highly preferred. BLS required.

  • Exhibits professionalism consistent with HW standards and policies with regard to performance, behavior and appearance
