PROGRAM DESCRIPTION

Care Management is a service model whereby all of an individual's caregivers communicate and interface so that the patient's needs are addressed in a comprehensive manner. This is done primarily through a "care manager" who oversees and provides access to all of the services an individual need to assure that they receive everything necessary to stay healthy, out of the emergency room and out of the hospital.

The Director of Care Management oversees the daily operations of the Care Management Department to ensure that children in foster care, prevention programs, and the community, their biological parents and foster parents, have the necessary resources, tools, and supports in place to effectively manage their healthcare and behavioral health needs in an efficient way.

MAJOR DUTIES & RESPONSIBILITIES:

  • Manages clinical operations of the Care Management Department, overseeing the care management services for children and adolescents
  • Works as the care coordination lead on the integrated health and behavioral health interdisciplinary team
  • Ensures departmental compliance with all regulator mandates, including that all children/adolescents served have all appropriate documentation in their charts.
  • Manages the department's required documentation ensuring that it is maintained in a timely and accurate manner through the effective utilization of the designated Health Homes Care Portal (Medicaid Analytics Performance Portal; MAPP) and Electronic Health Records (EHRs) as needed
  • Coordinates communication and collaboration with Foster Care and Prevention Programs to ensure each client's needs are met
  • Develops policies and procedures and continuous process improvement projects to identify gaps within system and streamline process
  • Develops and attends trainings related to Care Management as needed
  • Supervises Care Management Supervisors, who manage 4-5 Care Managers
  • Coaches, educates and mentors program staff on Care Management issues
  • Conducts timely performance reviews of direct reports
  • Implements, reviews, and revises Care Management policies and procedures that ensure program compliance
  • Demonstrates strong leadership and management skills to ensure progressive, positive development of Care Management department employees
  • Other duties as assigned as the program is implemented and develops
 
Position Requirements

QUALIFICATIONS:

  • Graduate Degree in Social Work, Nursing, Public Health, or related field, plus appropriate licensure.
  • Minimum of 10 consecutive years of prior administrative, clinical, and managerial experience
  • Demonstrated knowledge of best practices in the field of child welfare

EXPERIENCE:

  • Working knowledge of child welfare, developmental disabilities, mental health and primary health care fields
  • Experience managing case management teams
  • Knowledge of health homes and pending manage care initiatives for children/adolescents in foster care preferred
  • Experience using excel and electronic health care systems to review and manage data preferred Experience providing service coordination and information, linkages, and referrals for community-based services.
