POSITION TITLE: Psychiatric Nurse Practitioner

CGS Child/Adolescent Outpatient Mental Health Clinic

DEPARTMENT: Family Support Services

REPORTS TO: Director of Clinical Services & Strategic Initiatives

OVERVIEW OF POSITION:

The Nurse Practitioner (NP) is responsible to provide and coordinate the care of consumers in the ambulatory clinic setting by assessing, diagnosing, planning and intervening in actual and potential health problems, including evaluation of the consumer's response to the care.

Additional duties include but may not be limited to the ability to assess data reflective of the consumer's status and interpret the appropriate information needed to identify each consumer's requirements relative to his or her age, specific needs, and to provide the appropriate and applicable care as needed. This includes the provision of psychoeducation for the consumer and their supports; and collaboration with the clinic team members as applicable in planning for the consumer's care and achieving treatment goals.

DUTIES AND RESPONSIBILITIES:

  • Perform comprehensive physical assessments and document the consumer's biophysical and psychosocial status; as needed.
  • Review the consumer's prescribed medications and over-the-counter medications during health care encounters; and that medication supplies are maintained in sufficient quantity, are current and kept under proper storage (as administered at ambulatory setting, e.g., Injectable Rx).
  • Treats consumers by utilizing psychotherapeutic methods and medications; discussing progress toward goals with consumers; providing instructions; monitoring effect of medications; supervising staff provision of services.
  • Provide clinically proficient health screens and/or nursing assessment and health management (appropriate to Article 31 ambulatory clinic setting) for consumers; as needed.
  • Coordinate and monitor the health care needs of the consumers.
  • Identify the need for immediate or emergency treatment, and on-going medical needs as applicable.
  • Prepares evaluative reports, appropriate treatment plans and such additional documentation as required policy and provides relevant reports of the findings; participates in staff conferences and meetings.
  • Provides on-site crisis assessment and management of consumers.
  • Maintains interdisciplinary treatment by reviewing and approving treatment plans and progress; consulting and collaborating with primary care physicians, mental health therapists, nurses, and other health care providers as applicable.
  • Assures quality and safe service for consumers and staff by enforcing policies, procedures, standards, rules, Joint Commission for Accreditation of Health Care Organizations requirements, and legal regulations; participating in utilization reviews; remaining available for emergency consultations.
  • Improves staff competence by providing training and discussions regarding the interrelationship between psychosocial and physiological problems, psychotropic medications, crisis intervention, and related issues.
  • Maintains historical records by documenting symptoms, medications, and treatment events; writing summaries.
  • Updates job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
  • Perform other duties as requested.

QUALIFICATIONS:

  • Masters in Nursing (MSN)
  • Licensed to practice as a Nurse Practitioner (NP) in the State of New York; certified in psychiatric nursing
  • Knowledge of the Child Welfare System is desirable
  • Two years professional experience with emphasis on diagnostics and a team approach preferred
  • Ability to work some evening and weekend hours, including on-call consultation
  • Outstanding writing and communication skills
  • Proficiency in Spanish preferred
