With the belief that all people have the capacity to grow and change, we provide our services to people of all ages, cultures, races, religious preferences, genders, and sexual orientations in order to enhance wellness and recovery, thereby improving families, workplaces, and communities.   Position Behavioral Health Care Manager   Description Opportunity for an LSW/LPC to provide care management services to patients and families with moderately acute behavioral health needs or physical healthcare conditions that increase complexity.  The Care Manager will assess treatment patterns/utilization of services and work with case managers (and team leaders) around issues identified and changes in the treatment plans that support recovery and more positive outcomes.  The Care Manager will provide consultation to behavioral healthcare staff, as well as provide direct contacts with identified patients.      Position Requirements Qualified applicant must possess an LSW/LPC or commensurate skills and experience.    Location Southeast, Inc.
