Position Hospital Liaison  
Description

Exciting opportunity for an LSW/LPC to advocate and facilitate continued treatment to clients of Southeast while they are hospitalized at private psychiatric hospitals. The Assessment Specialist conducts intake assessments for community treatment teams, advocates on behalf of clients to facilitates collaborative discharge planning between the hospital social worker and the agency case manager and facilitates the initial linkage meeting between the client and the community treatment team manager.

Position Requirements

BA & LSW/LPC required; Master’s degree and LISW/LPCC preferred.
