Second Step Housing Behavioral Health Case Manager

The Position:

The Behavioral Health Case Manager will provide mental health assessment and connect residents with resources that promote housing retention, stability, sobriety and increased self-sufficiency. Family Service Agency is contracted with the Santa Barbara County Department of Behavioral Wellness to provide case management services to support the unique social service needs of individuals in the Prop 47 diversion program. Services are provided in the Good Samaritan offices, SB County Behavioral Wellness Detox facility, and/or in the community M-F between 8:30am-5:30 pm.

 

Working with those impacted by trauma can be fulfilling and meaningful, yet simultaneously exhausting and heart-rending. FSA strives to be a trauma-informed and resilience-oriented organization, actively identifying and employing methods to support staff to embody a healthy and sustainable balance of care for others and themselves.

 

Sample Job Duties

  • Provide case management services.
  • Provide and/or arrange crisis intervention services and mental health assessments and treatment planning.
  • Support residents in accessing substance abuse counseling/recovery and/or medical/mental health services and provide coordination of behavioral health and clinical services.
  • Collaborate with Santa Barbara County Department of Behavioral Wellness to ensure access to treatment for qualifying residents.
  • Provide advocacy/assistance for residents in obtaining public benefits.
  • Collaborate with Good Samaritan staff to address/resolve lease/compliance issues.
  • Provide and/or arrange for clients to develop/strengthen independent living skills.
 
Position Requirements

 

Employment Standards:

  • Must be one of the following: AMFT, ASW, APCC, LMFT, LCSW, or a LPCC.
  • Excellent clinical, interpersonal, analytical, computer, written, and verbal communication skills required.
  • Professional attitude, enjoyment of teamwork, and ability to maintain confidentiality a must.
  • Minimum of 2 years working with diverse communities, low-income clients, the homeless and formerly homeless, seniors, and the disabled preferred.
  • Must be able to work autonomously while also comfortable communicating with community and partner organizations.
  • Bilingual (English/Spanish) preferred.
  • This position requires a valid CA driver license and proof of insurance.

 

Classification, Hours and Pay Rate:

  • The position is full-time (40.0 hours), non-exempt, with benefits. The position also has the ability to start part-time, if needed, and grow to full-time.
  • Benefits include: medical, dental, vision, life insurance, and 403(b) retirement plan. Generous holiday, vacation, and sick leave available.
  • Pay is dependent on experience.
