Team Leader - Community Transition Program

Position Community Prison Re-Entry Program Team Leader  
Description

Opportunity to lead a team that assists individuals released from any Ohio DRC prison who re-enter Franklin County with needs for mental health and/or substance use disorder services. The Team Leader reviews prison information to better understand the background and needs of the individuals served and to assist with community-based service planning. The Team Leader and staff begin engagement with inmates while in prison to begin planning for successful re-entry. Forming community partnerships with parole officers, NetCare, homeless shelters, and re-entry housing organizations is a vital component of the Team Leader’s responsibilities. The Team Leader will ensure that Re-entry Specialists and Peer Specialists develop plans for treatment and supports to improve successful integration into the community and reduce prison recidivism. Data/outcome collection and reporting is essential for this program. The Team Leader will also become a member of a statewide Criminal Justice/Mental Health Group led by the Ohio Attorney General’s Office and will also work directly with Ohio Department of Mental Health and Addiction Services staff assigned to DRC prisons and referring to the Southeast program.

 

 

 
Position Requirements

Previous supervisory experience and experience with the criminal justice system preferred.
