The Case Manager will meet with inmates in the prison setting and ensure that they have transportation upon release, a place to say, and will provide a warm handoff to other Southeast and community programs, ensuring that the individual served is able to access the full range of services as quickly as possible.     Position Requirements AA/BA required; LSW preferred with Criminal Justice experience We are a healthcare facility and as such we require that our new employees are fully vaccinated (both COVID and FLU).
