We are seeking a RN/ LPN for a 1 to 1 private duty nursing position for a high medical care individual in their family home.

We are looking to fill the following shift: Friday - Monday - 8:30p - 8:30a - Parker

JOB DUTIES:

  • Skilled nursing care for hands on personal care, toileting, transferring
  • G-tube
  • Suctioning
  • Medication administration
  • Occasional medical appointments in the community

Individual Related Duties:

  1. Supervise and assist residents in planning and completion of routines such as medical care, resident self-care (dental, bathing, elimination, etc.), administrations of medications, treatments, dressing, and other nursing services, according to doctor instructions and condition of the individual.
  2. Maintains optimum health and well-being of the resident. Ensuring full participation and engagement of the resident, as per individual need and per their SP and ISSP, completing documentation as per agency guidelines.
  3. Implement and document Individual Service and Support plans, safety plans, and behavior management plans as stated, documenting after completion using progress notes and data collection forms specified per individual.
  4. Observes, evaluates, and records symptoms. Applies independent emergency measures to counteract adverse developments and immediately notifies physician/family/supervisor of resident’s condition. Document unusual behaviors/reactions in case file and using behavior tracking and/or incident report form as per agency guidelines.
  5. Meet individual nutritional needs by diet supervision and monitoring, according to doctor instructions and condition of the individual.
  6. Provide transportation to and from the community as per individual need and as assigned, including recreational activities, medical appointments, shopping, or day program as assigned.
  7. Participate as a member of Interdisciplinary Team, attending meetings and completing skills assessments as assigned, giving input regarding observations/recommendations of individual in services progress and needs.
  8. Assure that resident gets needed medical attention by documenting medication administration, familiarizing self with individual medical concerns, supervising appointments, documenting all medical visits with consultation sheets, and any medical concerns in individual notes as per agency guidelines, responding to emergency situations as per Emergency Data Form in individual in services file.
  9. Maintain in-house case and medical files, ensuring that all individual case and medical information is up to date. To include shift progress notes, vital signs documentation, ISSP/BISSP tracking, MAR, and medical appointment documentation.
  10. Assist with family requests in a polite and helpful manner reflective of agency philosophy, referring family questions or problems to supervisor and following through on requested actions within designated time.

Facility Related Duties:

  1. Provide guidance and assistance to residents to ensure that units are maintained in a clean and healthy manner, meeting guidelines on facility checklist on ongoing basis, and assisting with plan of correction in deficient areas. Notify supervisor and family of household health and safety maintenance needs.
  2. Conduct and document monthly fire drills using specified procedures and fire drill log, assisting residents in increasing independence in evacuation skills and general safety knowledge.
  3. Maintains medical equipment and supplies, conducting regular or as ordered cleaning. Notify supervisor and family of maintenance or supply needs.
  4. Maintain daily log and information exchange with other staff by recording events of the day and needed communication on daily log sheets, dating and signing each entry, and by reading and initialing log daily and at shift change. All entries are to be legible and professional in nature.

Agency Related Duties:

  1. Record actual hours worked daily on specified timesheet as per agency guidelines, have available for supervisor approval on last day of pay period or as otherwise directed. All leave requests must be submitted in writing and approved by supervisor prior to leave, as per agency guidelines.
  2. Record all mileage using specified forms as per agency guidelines and submit with time sheet on last day of pay period for approval.
  3. Attend staff meetings and scheduled inservice as requested by supervisor and meeting agency inservice requirements, completing all assigned tasks within designated time frames.

Expectations for Completion of Job Duties:

  1. Display professional appearance and behavior, interacting with other staff, individuals, parents, and service providers in a cooperative and friendly manner as observed by supervisor.
  2. Punctual and dependable at reporting to work. (Absenteeism and lateness do not result in adverse affects on the effective operation of the facility).
  3. Perform job duties with a positive attitude, responding to peak work period without loss of quality, accuracy, or thoroughness.
  4. Uphold individual rights by following individual in services rights and confidentiality guidelines and agency abuse, neglect, mistreatment, and exploitation policy daily.
  5. Provide positive role model to staff and individuals at all times, avoid use of profanity or derogatory slang, arrive to work in proper attire, model good problem solving and social interactions.

 

 
Position Requirements

GENERAL REQUIREMENTS: Active RN or LPN license. Previous experience in related field preferred. Must have valid drivers license, reliable vehicle, and proof of insurance, if applicable for setting. Motor vehicle report meeting driving requirements required prior to employment and are to be updated upon request, if in a driving position.

SUPERVISON RECEIVED: Works directly under supervision of the Program Manager; receives general guidelines and instruction from the Program Director. CEO available to resolve unusual work problems.
