The Outreach Coordinator engages with patients to coordinate referrals to immediate detox, addiction treatment programs, primary medical care and other needed services.    Position Requirements AA/BA degree and CDCA or higher required. Shift is every other Saturday & Sunday. As a healthcare provider, Southeast Healthcare is a non-smoking employer.  Staff members are required to be vaccinated for the flu and COVID.  Religious and medical exemptions may be requested during the hiring process.
