Description

JOB PURPOSE:

The Navigator position comprises is a combination of duties including Outreach, and Case Management. The person occupying this position will work directly with Veterans that are chronically homeless and assist them in accessing community support systems, services, benefits, and permanent housing. The Navigator directly supports the Homeless Veteran Community and collaborates with multiple service providers meeting regularly to triage, staff and house the most vulnerable unsheltered homeless Veterans.

Due to the target veteran population who experience a history of long term homelessness with significant needs and resistance to typical community services, the Navigator may be doing “all the work” and ‘carrying all the hope’ for the first 3-6 months of interaction. In other words, it will not be uncommon for the Navigator to establish a 6 month long Peer Support relationship before the Veteran will consider assistance with housing and other essential supportive services. A climate of trust must be prevalent between the two parties.

The Navigator will have a caseload of approximately 15 Veterans and serve as the primary advocate for the Veteran for 1-2 years.

ESSENTIAL JOB RESPONSIBILITIES:

15%-Conduct outreach and networking activities in the community; such as stand-downs, encampments, bus stations, shelters, under bridges etc. Must be active in and network at monthly Veteran community groups. Provide presentations at various agencies and for community partners within the greater metropolitan and surrounding area as needed. Promote the success and reputation of the Navigator program as well as the Supportive Services for Veteran Families program.

60%-Conduct initial eligibility screening of Veterans seeking program assistance. Complete initial intake and Case management documentation for eligible Veterans. Evaluate individual and family needs and include action plans to address goals. Complete service plan with Veteran including housing and other related needs. Coordinate and monitor services, including comprehensive tracking of Veteran activities in relation to service plan and Housing Inspections. Maintain appropriate records on each client, including HMIS; record of services provided with date/time of each Document detailed case notes. Prepare reports as requested by Program Director. Conduct home visits to follow up with the Veterans, and advocate.

10%-Assist Veterans with securing ID, personal documents, DD214, and other records needed to access services, benefits, housing, and employment.

10%-Meet regularly with Program Director to staff case load. Provide on-going program evaluation and recommendations to Program Director for continuous growth and quality.

5%-Perform other duties as assigned.

 
Position Requirements

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor’s Degree in Social Work, Sociology, Psychology or related field.

EXPERIENCE: 1+ year’s case management experience; 3+ years preferred. Experience working with the homeless veteran population with outreach efforts targeting the most vulnerable, unsheltered homeless Veteran households preferred. Knowledge of VA Programs, facilities, and community preferred.

LICENSES: LMSW, LBSW, LMFT preferred. Drivers License with clear record required.

VEHICLE: Must have daily use of a vehicle without prior notice. Must maintain current registration and current automobile liability insurance that is in compliance with Texas law. Must be available and willing to transport clients to various locations and with such frequency as the business need dictates. Mileage reimbursement provided.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion. Must be available during the stated service hours.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
