Job Purpose:

Point of contact for care staff for the shift. Carrying out Supervisory responsibilities in accordance with organizational policies and procedures and regulatory requirements and applicable law. Demonstrates problem solving and decision making abilities. Ability to manage resident care with specific knowledge and hands on experience and understanding of dementia and behaviors, bedside nursing, symptom management, crisis intervention and family intervention. Demonstrated management ability with an interdisciplinary team and other health care providers. Excellent understanding of Dementia, skills in communication both verbal and written, interpersonal relations, documentation, organization. Ability to develop and maintain rapport with residents, families and staff.

Essential Duties and Responsibilities:

  • Currently licensed as a Practical Nurse (LPN) must be in good standing with the State Board of Michigan
  • Basic Life Support (BLS)
  • Demonstrated clinical competence as a staff nurse in acute care hospital, or other Dementia Care facility for a minimum of 2 years.
  • Effective verbal, written, and listening skills.
  • Effective organizational/priority setting skills
  • Ability to effectively communicate with co-workers, residents (geriatric, dementia patients), visitors, medical staff, pharmacy and hospital staff as necessary.
  • Ability to be an EFFECTIVE team member, as well as a team builder, offering input, willingness to trust the skills and expertise of other team members, share in group decision making, and refer them to appropriate resources of help if indicated.
  • Conflict management, problem solving and team building skills
  • Calming, level-headed approached when faced with emergency situations,
  • Ability to assess resident's physical, psychosocial, emotional, cultural and spiritual needs.
  • In the absence of Nurse Manager, serves as clinical nursing resource and mentor to care staff and new orientees.
  • Administers medications and treatments as prescribed within scope of nursing license, and our license.
  • Provides resident care as needed, including documentation
  • Acts as a resource to assure safety and outstanding quality care of residents.
  • Conducts disciplinary actions as necessary in compliance with policies.
  • Completes designated requirements (forms, software entry) related to resident care health status, staffing and scheduling.
  • Attends all staff meetings, in-service programs, safety courses, and other required meetings and trainings.
  • Stays up to date and knowledgeable of all departmental communication.
  • Actively supports our vibrant life culture.
