Share Email Opening Title Correctional Re-Entry Specialist  Position Correctional Re-Entry Specialist   Full-Time/Part-Time Part-Time   Shift Various Shifts   Number of Openings 2   Location Forest County   Open Date 1/17/2018   Close Date 2/17/2018   Description Matt Talbot Recovery Services, Inc., sister company of Horizon Healthcare, is currently recruiting for Part Time Re-Entry Specialists to provide long-term, community based services to convicted WI Ch. 980 correctional offenders following their release and transition back to their home community. Working cooperatively with the DOC, DHS, and Supervised Release Teams, the Re-Entry Specialist is responsible for transporting clients from their home residence to designated locations for the purpose of treatment, accessing community services, employment purposes, religious beliefs, or caring for their basic living needs. The Specialist must follow specific reporting protocols using their assigned smart phone during transports and most importantly, maintain direct supervision of the client at all times for the duration of the outing to ensure rule compliance. Example outings include attending school, work, medical appointments, job search, grocery shopping, haircuts, and SOT treatment. Responsibilities: Provide timely transportation of the client to and from approved destinations. Maintain continuous, direct supervision of the client as directed by DOC and/or DHS. When direct supervision is not required or permitted, follow their direction to maintain the appropriate level of continuous observation of the client and/or area. Chaperone in-home visits a client may have with contacts approved by the Supervised Release team. Communicate with the necessary entity at the start and finish of every transport or service. Document all services within electronic client record system before, during, and immediately after each transport or service assigned Inspect premises completely and respectfully during assigned random residence monitoring assignments Report all violations according to procedures Serve as a role model and at times a coach for the client. For example, coaching a client on making cost effective and healthy purchases at a grocery store or coaching a client on how to check in at an appointment, coaching a client on how to use a computer at a job center, and demonstrating appropriate community behavior   Position Requirements Requirements: One (1) year experience providing services to one or more of the following populations: Individuals who have committed a sexual offense Individuals with serious and persistent mental illness Individuals who have committed violent crimes Individuals who have a developmental disability Maintain Valid WI Driver's License free of moving violations and/or at fault accidents in the past two years A criminal background check may be required for this position subject to Wisconsin law.
