Description

The position of Medical Case Manager(MCM) and Supervisor of Clinical Track is exempt.  It is the responsibility of the MCM to perform the following duties for a minimum of 30-60 clients:

 

Tasks and Responsibilities

The MCM/Supervisor of Clinical Track will work in conjunction with a medical provider as a team to provide case management to patients who are in the provider’s care.  The MCM/Supervisor of Clinical Track is responsible for doing individual clinical supervision for up to 6 case managers who are working toward their clinical licensure, for one hour per week per Person supervised. Case load will be flexible to allow for supervision. Candidate must be eligible to supervise candidates for their clinical licensure in the state of Pennsylvania. The MCM/Supervisor of Clinical Track and care team will meet on a daily basis to discuss the medical/service care plan for each client. The medical/service care plan will include:

 

  1. The development of the bio-psychosocial assessment of each client under his/her care.
  2. The development of a Plan of Action to be implemented by the client and the MCM/Supervisor of Clinical Track.
  3. Maintenance of a client case record and the development of a Service Care Plan to be reviewed every six months or more frequently if necessary.  Included in the items to be assessed are:
  4. Emergency, medical, housing and legal needs; skills; and quality of life goals.
  5. Proof of eligibility for services.
  6. Documentation of adherence to medical appointments and medical care plan, including HIV medication adherence.

 

Service Coordination

  1. The MCM/Supervisor of Clinical Track is responsible for assuring that services required by the client are coordinated in a fashion which assists the client in meeting long-term and short-term goals.
  2. The MCM/Supervisor of Clinical Track will therefore maintain current information on available services to clients living with HIV/AIDS in Philadelphia.
  3. The MCM/Supervisor of Clinical Track will be responsible for making quick decisions in the event that a crisis occurs; therefore he/she must be easily accessible to the client.

 

Training

Twenty hours of professional education and training will be required on an annual basis as described by AACO and TPAC.

 

Record Keeping

The MCM/Supervisor of Clinical Track is responsible for maintaining all documentation through entry into CareWare related to the delivery of case management services to the client in a client record, which is confidential and kept in a locked file cabinet. 

The MCM/Supervisor of clinical Track is responsible for maintaining all records related to statistical reports in CareWare required by the agency and the funders to evaluate service performance, as well as records utilized for continuous quality improvement projects.

 

 

Ongoing Requirements

  1. Develop a resource file
  2. Coordination with other agencies providing like case management services
  3. Compliance with additional standards
  4. Bi-weekly supervision
  5. Monthly case management meetings

 

Hours of Work

The MCM/Supervisor of Clinical Track will work 40 hours per week

 

Accountability

For all medical issues the MCM/Supervisor of Clinical Track will consult with the medical provider.  The MCM/Supervisor of Clinical Track will also report to his/her team member for service coordination plans for their patients. The MCMSupervisor is responsible to the Case Management Supervisor for vacations, time sheets, and reports.

 

Supervisory Responsibility

The MCM/Supervisor of Clinical Track will supervise the Medical Case Managers who are working toward their clinical designation in Social Work.

 
Position Requirements

Skills, Knowledge and Experience

Candidates must be Licensed and have a Clinical Social Work degree (LCSW). He/she must know and be able to use the local service network, including but not limited to all HIV services in the Philadelphia region.
