The position of Medical Case Manager is exempt.

It is the responsibility of the Medical Case Manager to perform the following duties for a minimum of 230 clients throughout the period of one year:


Tasks and Responsibilities

The Medical Case Manager will work in conjunction with a medical provider as a team to provide case management to patients who are in the provider’s care.  The Medical Case Manager and provider will meet on a daily basis to discuss the medical/service care plan for each client.

  1. The development of the bio-psychosocial assessment of each client under his/her care, including the acuity scale.
  2. The development of a Plan of Action to be implemented by the client.
  3. Maintenance of a client case record and the development of a Service Care Plan to be reviewed every three months or more frequently if necessary.  Included in the items to be assessed are:
  4. Emergency, medical, housing and legal needs; eligibility; skills; and quality of life goals.
  5. Housing applications through AACO.
  6. The Medical Case Manager will team with a medical provider. They will meet daily to discuss the scheduled appointments of the day and come up with a treatment plan for the patient.
  7. The Medical Case Manager and the medical provider will meet weekly for case conferences.

Service Coordination

  1. The Medical Case Manager is responsible for assuring that services required by the client are coordinated in a fashion, which assists the client in meeting long-term and short-term goals.
  2. The Medical Case Manager will therefore maintain current information on available services to clients living with HIV/AIDS in Philadelphia.
  3. The Medical Case Manager will be responsible for making quick decisions in the event that a crisis occurs; therefore he/she must be easily accessible to the client.


Twenty hours of professional education and training will be required on an annual basis as described by AACO and TPAC.

Record Keeping

The Medical Case Manager is responsible for maintaining all documentation through entry into CareWare related to the delivery of case management services to the client in a client record, which is confidential and kept in a locked file cabinet. 

The Medical Case Manager is responsible for maintaining all records related to statistical reports in CareWare required by the agency and the funders to evaluate service performance, as well as records utilized for continuous quality improvement projects.

Ongoing Requirements

  1. Develop a resource file
  2. Coordination with other agencies providing like case management services
  3. Compliance with additional standards
  4. Bi-weekly supervision
  5. Monthly case management meetings


Hours of Work

The Medical Case Manager will work 40 hours per week



For all medical issues the Medical Case Manager will consult with the medical provider.  The Medical Case Manager will also report to his/her team member for service coordination plans for their patients. The Medical Case Manager is responsible to the Director of Client Services for vacations, time sheets, and reports.


Supervisory Responsibility

The Medical Case Manager has no supervisory responsibility


Position Requirements

Skills, Knowledge and Experience

The Medical Case Manager will have demonstrated experience in the field and a Masters degree in social work, psychology, sociology or behavioral health. He/she must know and be able to use the local service network, including but not limited to all HIV services in Philadelphia region. The Medical Case Manager must be license eligible.
